摘 要:目的:探讨急性缺血性脑卒中伴不同部位脑微出血危险因素之间的相关性,为伴有CMBs的缺血性卒中急性期治疗提供新证据。方法:回顾性分析在某科住院诊断为急性脑梗死的患者,根据SWI扫描CMBs分为无、有CMBs两组;进一步按CMBs解剖位置分为大脑皮层、皮质下、基底节及混合组,比较有无CMBs及其不同位置CMBs与危险因素之间的相关性;Logistic回归模型分析测定血压血糖血尿酸与脑微出血CMBs的独立相关性。结果:共336例患者最后纳入研究,其中182例患者有CMBs,154例患者无CMBs,CMBs检出率为54.2%;182例CMBs患者中大脑皮层组37例(20.3%),皮质下45例(24.7%),深部基底节组61例(33.5%),混合组39例(21.4%),4组脑微出血CMBs的发生率比较差异无统计学意义(P>0.05),与无CMBs组相比,有CMBs的患者血压血尿酸值更高,差异均有统计学意义(P<0.05)。结论:血清高尿酸水平和高血压是急性缺血性脑卒中患者CMBs的独立相关因素。Objective To explore the correlation between acute ischemic stroke with micro-hemorrhage risk factors in different parts of the brain and provide new evidence for the treatment of acute ischemic stroke with CMBs. Methods: Retrospective analysis was performed on patients diagnosed with acute cerebral infarction in our department. Divided them into with and without CMBs group according to SWI scanning CMBs.Further divided into the cerebral cortex, subcortex, basal ganglia and mixed group according to the anatomical location of CMBs, and compared the correlation between the presence or absence of CMBs and the different locations of CMBs and risk factors. The independent correlation between blood pressure, blood glucose, uric acid and cerebral microhemorrhage (CMBs) was analyzed by Logistic regression model. Results: A total of 336 patients were included in the study, including 182 patients with CMBs, 154 patients without CMBs, and the detection rate of CMBs was 54.2%. Among the 182 CMBs patients, 37 cases were from the cerebral cortex group (20.3%). There were 45 cases under cortex (24.7%) and 61 cases in deep basal ganglia group (33.5%). 39 cases (21.4%) in mixed group; There was no statistically significant difference in the incidence of CMBs in the 4 groups ( P 〉0.05). Compared with the non-cmbs group, patients with CMBs had higher blood pressure uric acid value, and the differences were statistically significant ( P 〈0.05). Conclusion: High serum uric acid level and hypertension are independent correlative factors of CMBs in patients with acute ischemic stroke.
