内蒙古乌海市结直肠癌不同年龄段临床表现与病理特征比较

摘　　要：目的:探索不同年龄段结直肠癌临床表现与病理特征的差异。方法:以内蒙古乌海地区2007-01~2011-12月确诊结直肠癌并明确病理类型的住院、门诊患者854例为研究对象,将患者按年龄划分青年、中年及老年3个年龄段组,比较三组结直肠癌患者临床表现及病理特征差异。结果:3组患者以便血、腹泻、腹痛为主要症状且便血比例最高(41.0%);青年组恶性程度高的比例高于中、老年组(P<0.05),年龄与直肠癌恶性程度呈负相关(rs=-0.105,P<0.01)与TNM呈正相关(rs=0.08,P<0.05),检查出的患病年龄越小,恶性程度越高;年龄越大,肿瘤进展程度越高,预后越差。结论:加强对不同年龄段人群直肠癌的健康教育,加强行为干预,制定防治策略,降低高危人群发病危险;增加对人群的体检筛查,以期早发现、早治疗,提高居民健康水平,降低肿瘤进展程度,提高预后效果。Objective： To explore clinical manifestation and pathological features among different colorectal cancer age groups. Methods： This article takes 854 inpatients and out patients with colorectal cancer who were confirmed and clear pathological type. Those patients were divided into three groups： young age, middle age, grey age and then clinical manifestation and pathological features were compared among three age groups. Results：Blood in the stool, diarrhea, abdominal pain were main symptoms, while blood in the stool constitute 41.0%. Percentage of high--grade cancer of young age group is highest among three groups. Patients＇ age and malignancy of rectal cancer was negatively correlated （r, =--0. 105, P 〈0.01） and positive correlated with TNM （Classification of Malignant Tumours） （r, = 0. 08,P〈0. 05）. When patients check out cancer, the younger the higher degree of malignancy; the older the higher degree of tumour progression and worse prognosis. Conclusion：It is necessary to increase health education and behaviour interventions among different age groups. Developing prevention and control strategies to decrease high-risk groups＇ dangerous risk. Increasing physical examination screening to early detection, early treatment, increase residents＇ health level, decrease turnout progression and improve prognosis.
