摘 要:目的探讨多肿瘤抑制基因1(P16)、生存素(Survivin)和基质金属蛋白酶(MMP-9)在宫颈病变中的表达及其临床意义。方法以140例宫颈鳞癌及CIN组织为研究对象,同时选取32例正常宫颈及慢性宫颈炎组织为对照组,采用免疫组化SP法检测P16、Survivin和MMP-9在各组中的表达。结果 P16、Survivin和MMP-9在CIN组中的阳性表达明显高于对照组,差异有统计学意义(P<0.05)。在CIN组中,随着病变的进展三者的阳性表达率升高,差异有统计学意义(P<0.05)。单独检测P16、Survivin及MMP-9的敏感度为85.29%、79.41%、83.82%,特异度分别为52.88%、57.69%、52.88%;3指标联合检测的敏感度为94.10%,特异度为53.38%。结论 P16、Survivin和MMP-9随着宫颈病变的进展阳性表达逐渐升高,在宫颈癌组织中呈高表达,三者联合检测对早期判断宫颈病变性质有重要价值。Objective Through detecting the expressions of P16,Survivin and MMP-9 in cervical lesions,to explore their clinical significance. Methods 140 cases of cervical squamous cell carcinomas and CIN tissue were selected as the research objects,32 cases of normal cervix and chronic cervicitis tissues were selected in the control group,and the expressions of P16,Survivin and MMP-9 in each group were detected by using immunohistochemical SP method. Results The positive rates of P16,Survivin and MMP-9 in experimental group were higher than in control group,and the differences were statistically significant( P 〈 0. 05). In the CIN group,the positive rate of the three increased with the progress of the lesion,and the differences were statistically significant( P 〈 0. 05). The sensitivity of P16,Survivin,and MMP-9 alone was 85. 29%,79. 41%,and 83. 82%,and the specificity was 52. 88%,57. 69%,52. 88%. The combined sensitivity of the three indexes was 94. 10%and the specificity was 53. 38. %. Conclusion The positive expressions of P16,Survivin and MMP-9 increased with the progression of cervical lesions gradually,and it was highly expressed in cervical cancer. The joint detection of P16,Survivin and MMP-9 plays an important role in the early diagnosis of cervical lesions.
