食管钡餐造影联合CT评价食管癌患者放疗后近期疗效的价值

摘　　要：目的探讨食管钡餐造影联合CT综合评价食管癌患者放疗后近期疗效的价值。方法选取2012年1月至2012年12月间宝鸡市中心医院收治的62例食管癌放疗患者,按照不同近期疗效评价方法分为造影组与联合组,每组31例。造影组患者采用食管钡餐造影评价法,联合组患者采用食管钡餐造影联合CT综合评价法,比较两组患者的临床疗效和肿瘤转移率。结果造影组患者随访1、3和5年的临床有效率分别为74.2%、48.4%和35.5%,联合组患者分别为83.9%、61.3%和45.2%,联合组均高于造影组,差异均有统计学意义(均P<0.05)。造影组患者随访1、3和5年的肿瘤转移率分别为35.5%、58.1%和74.2%,联合组分别为16.1%、38.7%和48.4%,联合组均低于造影组,差异均有统计学意义(均P<0.05)。结论食管钡餐造影联合CT综合评价食管癌患者放疗近期疗效的准确率较高,助于指导后续治疗,值得推广。Objective To evaluate significance of barium esophagography combined with CT in the evaluation of short-term efficacy of radiotherapy for esophageal cancer. Methods A total of 62 patients with esophageal cancer on radiotherapy at Baoji Central Hospital were selected from January 2012 to December 2012. According to the different efficacy evaluation methods, patients were divided into the contrast group and the combination group, with 31 patients in each group. The contrast group received esophagogram eval- uation method, and the combination group used esophageal barium meal combined with CT comprehensive e- valuation method. Clinical efficacy and tumor metastasis rate were compared between the two groups. Results The 1-, 3-and 5-year efficacy rate was 74. 2%, 48.4% and 35.5% for the contrast group and 83.9%, 61.3% and 45.2% for the combination group with the combination group higher than the contrast group （all P 〈0.05）. The 1-, 3-and 5-year tumor metastasis rate was 35.5%, 58.1% and 74. 2% in the contrast group and 16. 1%, 38.7% and 48.4% , respectively for the combination group with, the latter low- er than the former （ all P 〈 0. 05）. Conclusion Barium esophagography combined with CT has a high ac- curacy in evaluating the short-term efficacy of radiotherapy for esophageal cancer. It is helpful for guiding the subsequent treatment, and is worthy of popularization and application.
