临床护理路径联合快速康复外科理念对胃癌根治术后患者的干预效果

摘　　要：目的探讨临床护理路径(CNP)联合快速康复外科理念(FTS)对胃癌根治术后患者干预效果。方法选取2016年12月至2018年3月间江苏省扬州大学附属医院收治的接受胃癌根除术治疗的86例胃癌患者,采用随机数表法将患者分为研究组和对照组,每组43例。研究组患者采用CNP联合FTS护理干预,对照组患者采用常规围术期护理干预。结果研究组患者知识知晓总分高于对照组患者,差异有统计学意义(P<0.05)。研究组患者术后肛门排气时间和住院时间均短于对照组患者,差异均有统计学意义(均P<0.05)。研究组患者炎性因子中C反应蛋白(CRP)、血清淀粉样蛋白(SAA)和IL-6变化水平均优于对照组患者,差异均有统计学意义(均P<0.05)。研究组患者的并发症总发生率为18.6%,低于对照组患者的41.9%,差异有统计学意义(P<0.05)。结论胃癌根治术治疗过程当中,采用CNP联合FTS护理干预效果较好,临床上应进一步推广应用。Objective To explore the efficacy of clinical nursing pathway （CNP） combined with fast-track surgery （FTS） guided nursing in postoperative radical gastrectomy and its effects on stress re- sponse. Methods A total of 86 patients who underwent adical gastrectomy for gastric cancer at Yangzhou University Affiliated Hospital from December 2016 to March 2018 were selected and randomly divided into a research group and a control group with 43 patients in each group. The research group received CNP com- bined with FTS guided nursing intervention and the control group received routine perioperative nursing in- tervention. Results The overall knowledge awareness socre was high in the research group than in the con- trol group （ P 〈 0.05 ）. The anal exhaust time and hospital stay was shorter in the research group than in the control group （all P 〈0.05 ）. Changes in inflammatory factor C reactive protein （CRP）, serum amyloid a protein （ SAA）, and IL-6 were significantly better in the research group than in the control group （ all P 〈 0.05 ）. The overall complication rate was 18.6% in the research group which was lower than 41.9% of the control group （P 〈 0. 05）. Conclusion The efficacy of CNP combined with I;TS guided nursing in post- operative radical gastrectomy is high and should be further popularized in clinical practice.
