摘　　要：目的 分析腮腺良性肿瘤的发病特点及CT表现，提高临床诊断及鉴别诊断水平。方法 回顾性分析经手术及病理证实的腮腺良性肿瘤患者64例，均行CT平扫及增强扫描。结果 本组病例包括腮腺多形性腺瘤36例，腮腺腺淋巴瘤13例，腮腺基底细胞腺瘤4例，腮腺肌上皮瘤4例，腮腺淋巴上皮样囊肿4例，腮腺脂肪瘤2例，腮腺海绵状血管瘤1例。腮腺良性肿瘤以多形性腺瘤最为多见，表现为特征性的渐进性强化；腺淋巴瘤次之，为特征性的“快进快出”；基底细胞腺瘤为双期均显著强化；淋巴上皮样囊肿为环形持续强化；海绵状血管瘤为明显均匀强化；脂肪瘤呈脂肪密度，结合特殊表现如囊变、钙化、出血等有助于鉴别诊断。结论 腮腺良性肿瘤具有一定特征性的CT表现，结合患者发病年龄、性别、部位和临床等特点，可以提高其诊断准确率。Objective To analyze the characteristics and CT performance of benign parotid tumor to improve the clinical diagnosis and differential diagnosis.Methods 64 cases of benign parotid tumors which were proved by operation and pathology were analyzed retrospectively.All of them received plain and enhanced CT scan.Results This group includes 36 cases of pleomorphic adenoma parotid,13 cases of parotid adenolymphoma,4 cases of parotid basal cell adenoma,4 cases of parotid epithelioma,4 cases of parotid lymphoepithelioid cyst,2 cases of parotid lipomyoma and 1 case of parotid cavernous hemangioma. Pleomorphic adenoma, characterized by progressive enhancement, was the most common kind of benign parotid tumors.The secondary kind was the adenolymphoma, characterized by fast in fast out.Basal cell adenoma was significantly enhanced in both stages.The lymphoidepithelioid cyst was continuously strengthened in a ring. Cavernous hemangioma was uniformly enhanced as vessels.The lipomyoma presents fat density,combing with special performances, such as cyst degeneration, calcification, hemorrhage,which was contribute to the differential diagnosis. Conclusion Benign parotid tumor has certain characteristics of CT performance, combing with the features of onset age, gender, location and clinical, that can further improve diagnosis accuracy.
