摘 要:目的 分析颅底软骨肉瘤的CT、MRI影像及病理资料,总结其影像学特征。方法 收集经临床及病理证实的颅底软骨肉瘤13例, 其中10例行MRI平扫及增强扫描,10例行CT平扫及增强扫描,7例同时行CT、MRI扫描。影像特征由2名分别有8年和15年影像诊断经验的影像医师共同分析确定,病理诊断由1名有10年病理诊断经验的病理医师确诊。结果 本组颅底软骨肉瘤均位于颅底骨缝连接处,其中蝶岩缝7例,岩枕缝3例,额蝶缝2例,蝶枕缝1例。颅底软骨肉瘤影像学征象:CT均见溶骨性骨质破坏,平扫密度不均匀,瘤内散在环状钙化,钙化边缘模糊,MRI T2WI以高、低混杂信号为主,瘤体边缘均呈分叶状,延迟强化是其特征。结论 CT图像上瘤内环状模糊钙化、肿瘤实质的延迟强化及特殊的生长部位(骨缝连接部)有助于颅底软骨肉瘤的诊断与鉴别。Objective To analyze the image characteristics of chondrosarcomas in the skull base on CT and MRI and their pathological findings, and then summarize the image features.Methods 13 chondrosarcomas of the skull base confirmed by clinical and pathology were collected.10 cases were performed by MRI plain and enhanced scanning, 10 cases by CT plain and enhanced scanning and 7 cases by both CT and MRI. The image features were determined by two imaging doctors respectively (8 and 15 years with the experiences of imaging diagnosis).The pathological diagnoses were determined by a pathologist (10 years with the experiences of pathological diagnosis).Results All the chondrosarcomas of the skull base in this study were located at the suture areas of the skull base,including 7 cases at spheno-petrosal suture,3 at petro-occipital suture,2 at fronto-sphenoid suture and 1 at spheno-occipital suture.The image features of chondrosarcomas of the skull base on CT and MRI in this study included osteolytic destruction of bone, uneven density on CT plain scan,interspersed ringlike calcifications with blurred margins within the tumor, high and low hybrid signals in tumor on MRI T2 WI, the lobulated margin of tumor, and delayed enhancement of tumor parenchyma on both CT and MRI enhanced scanning. Conclusion Ringlike calcifications with blurred margins within the tumor on CT images,delayed enhancement of tumor parenchyma and the special location of growth (the area of bone suture) may contribute to the diagnosis and identification of chondrosarcoma of the skull base.
