摘　　要：目的 探讨妊娠滋养细胞肿瘤的MRI表现特点。方法 回顾性分析21例经组织病理学及临床随访证实的妊娠滋养细胞肿瘤患者的MRI表现。结果 20例子宫不同程度增大。病灶全部位于子宫，16例位于子宫肌层或侵犯子宫全层，5例位于宫腔并局部侵犯肌层。病灶呈囊实性16例，其中呈蜂窝状7例。18例肿块相邻结合带破坏。15例肿瘤内伴有出血。16例宫旁或肌层内见流空血管影。病灶T1WI呈低信号14例，T2WI呈高信号或混杂信号19例。增强肿块范围显示清晰，肿块囊性部分不强化，分隔及实性部分强化。结论 妊娠滋养细胞肿瘤病灶呈蜂窝状及合并血管流空影具有特征性。MRI能很好显示病变的位置、大小、范围及侵犯肌层状况，结合临床病史及血HCG可提示诊断。：Objective To investigate MRI features of gestational trophoblastic neoplasia. Methods The MRI appearances of 21 cases of gestational trophoblastic neoplasia confirmed by histopathology and clinical follow-up results were analyzed retrospectively. Results Uterus was enlarged to different extent in 20 cases. Gestational trophoblastic neoplasia were all in the uterus, 16 cases were located in the myometrium of the uterus or the whole uterus, 5 cases showed with myometrium and intrauterine mass. 16 cases showed with cystic-solid mass,of which 7 cases showed characteristic Honeycomb-like appearance.The junctional zone was disrupted in 18 cases with gestational trophoblastic neoplasia.15 cases showed with intratumoral hemorrhage.Flow void signals of myometrial and para-uterine vessels were detected in 16 cases. There were 14 cases of low signal in T1WI and 19 cases of high signal or mixed signal in T2 WI. The enhanced scan showed the range of the mass more clearly, cystic part of the mass were not enhancedand, and showed septal enhancement and enhancement of the solid part.Conclusion Gestational trophoblastic neoplasia showed with the characteristic Honeycomb-like appearance manifestation and flow void signals.MRI can be used to show the location, size, scope and the status of myometrium infiltration,it can be diagnosed by combining clinical history and blood human chorionic gonadotropin （HCG）.
