肿瘤标志物联合检测对妇科肿瘤的诊断价值

摘　　要：目的探讨CA125、CA199、CA242、CEA联合检测在妇科肿瘤临床诊断中的应用价值。方法用C12多肿瘤标志物蛋白芯片检测系统联合检测95例妇科恶性肿瘤患者和167例妇科良性疾病患者血清中的12种肿瘤标志物水平,以149例健康体检女性为对照组,分析其中四种肿瘤标志物CA125、CA199、CA242和CEA在三组人群中的阳性率,计算联合检测四种肿瘤标志物对妇科肿瘤诊断的敏感性和特异性。结果妇科恶性肿瘤患者血清中CA125、CA199、CA242和CEA四种肿瘤标志物的水平明显高于良性疾病患者和健康人群,差异有统计学意义(P <0. 05);四种肿瘤标志物在各组人群的单独检测中阳性率是:恶性肿瘤组>良性疾病组>对照组(P <0. 05),其中又以CA125阳性率最高,恶性组与良性组肿瘤标志物联合检测阳性率高于CA199、CA242、CEA单独检测(P <0. 05),而CA125在两组的单独检测与联合检测阳性率相差不大(P> 0. 05);恶性肿瘤组CA125单独检测和联合检测比较,敏感性和特异性都相差不大(P> 0. 05),在卵巢癌患者中CA125无论单独检测还是联合检测,敏感性比恶性组高(P <0. 05),特异性相差不大(P> 0. 05)。结论肿瘤标志物联合检测可提高妇科恶性肿瘤诊断的灵敏性和准确性,有助于指导临床治疗。Objective To explore the value of CA125, CA199, CA242 and CEA combined detection in clinical di- agnosis of gynecological tumors. Methods With a combined C12 nmltiple tumor markers protein chip detection system de tection, 95 cases of gynecologic malignant tumor patients and 167 patients with benign disease of department of gynaecology 12 tumor markers in the serunl level of physical examination in 149 cases of healthy women as control group, the analysis of four kinds of tumor marker CA125, CA199, CA242 and CEA in the positive rate of the three groups, calculate the joint de tection of four tumor markers sensitivity and specificity of gynecology tumor diagnosis. Results The levels of CA125, CA199, CA242 and CEA in serum of patients with gynecological malignant tumors were significantly higher than those of pa- tients with benign diseases and healthy populations, and the difference was statistically significant （ P 〈 0.05 ）. Four kinds of tumor markers in all groups separate testing positive rate is ： malignant tumor group 〉 benign disease group 〉 normal group （ P 〈 0.05 ）, and with CA125 positive rate is highest, malignant group and benign tumor markers combined detection positive rate higher than that of CA199, CA242, CEA detection alone （P 〈 0.05） , and CA125 in two groups of single detection and joint test positive rate were similar （P 〉 0.05 ） ; Detection of CA125 separate malignant tumor group and the joint detection, sensitivity and specificity- were similar （P 〉 0.05 ） , in patients with ovarian cancer CA125 detection alone or combined detec- tion, sensitivity is higher than malignant group （ P 〈 0.05 ） and specificity were similar （ P 〉 0.05 ）. Conclusion Combined detection of tumor markers can improve the sensitivity- and accuracy of gynecological malignant tumor diagnosis and guide clin- ical treatment.
