摘 要:目的:研究子宫内膜组织hTERC基因表达的情况,探索hTERC基因在子宫内膜癌发生发展中的关系。方法:收集正常子宫内膜、子宫内膜增生症(单纯型增生、复杂型增生、不典型增生)及子宫内膜癌组织石蜡切片各25例,共125例标本行hTERC基因FISH检测。结果:子宫内膜正常组、子宫内膜单纯型增生组、子宫内膜复杂型增生组、子宫内膜不典型增生组及子宫内膜癌组织中hTERC基因阳性表达率分别为20%(5/25)、52%(13/25)、28%(7/25)、20%(5/25)、36%(9/25),子宫内膜单纯性增生阳性率高于正常组和不典型增生组,差异统计学有意义(χ~2=5.5556,P=0.0184),子宫内膜癌组阳性率高于子宫内膜正常组,但经检验两组hTERC基因扩增阳性率没有统计学差异(χ~2=1.5873,P=0.2077)。按子宫内膜癌临床分期,I/II期hTERC基因阳性率为40%(8/20)、III/IV期20%(1/4),虽然差异没有统计学意义(P=0.6206),但临床观察hTERC基因在早期临床期别有高阳性扩增。按子宫内膜样腺癌细胞分化程度,高分化hTERC基因阳性率22.22%(2/9)、中分化50%(4/8)、低分化37.5%(3/8),hTERC基因随着腺癌分化程度越差扩增阳性率增高,但经Fisher检验没有统计学差异(P=0.5165)。结论:本研究通过FISH检测子宫内膜组织hTERC基因表达情况得出,子宫内膜单纯性增生阳性率最高,子宫内膜癌患者较正常子宫内膜组织hTERC基因有扩增趋势,并随着癌细胞低分化hTERC基因扩增增加,提示子宫内膜恶性病变可能与hTERC表达异常相关,hTERC基因表达异常预测子宫内膜病变风险增高,可指引临床病情观察诊治。Objective To study the expression of hTERC gene in endometrial tissue, and to explore the role of hTERC gene in endometrial carcinogenesis. Methods:To collect 25 cases each in normal endometrium, endometrial hyperplasia (simple hyperplasia, complex hyperplasia, atypical hyperplasia) and endometrial carcinoma from 2015 to 2016, hTERC gene was detected in 125 specimens by Fluorescence in situ hybridizatio(FISH)technique. Results:The positive rate of hTERC gene in normal endometrium,simple endometrial hyperplasia,complex endometrial hyperplasia,atypical endometrial hyperplasia and endometrial carcinoma were respectively 20% (5/25),52% (13/25),28% (7/25),20% (5/25),36 % (9/25 ),the positive rate in simple endometrial hyperplasia was significantly higher than in normal endometrium group and atypical endometrial hyperplasia group(χ 2 =5.5556, P =0.0184).The positive rate in endometrial carcinoma was higher than in normal endometrium group, but the two groups by chi square test hTERC gene amplification positive rate difference was not statistically significant(χ 2 =1.5873, P =0.2077). According to the clinical staging of endometrial cancer, the positive rate of hTERC gene in I /II stage and III /IV stage were respectively 40% (8/20), 20% (1/4), the clinical early stage hTERC gene amplification positive was higher than that of advanced cases, but after Fisher exact probability calculation, and there was no significant difference( P =0.6206).In endometrioid adenocarcinoma differentiation, high differentiation, differentiation and low differentiation of hTERC gene positive expression rate was 22.22% (2/9), 50% (4/8), 37.5% (3/8), with the de gree of differentiation of adenocarcinoma is poor, the positive rate of hTERC gene amplification increased, but after Fisher exact probability calculation method no statistical significance( P =0.5165).Conclusion:Simple endometrial hyperplasia in patients with the highest hTERC gene positive rate, endometrial cance
