血清可溶性转铁蛋白受体测定在地中海贫血复合铁缺乏诊断与治疗中的意义

摘　　要：目的探讨血清可溶性转铁蛋白受体水平(soluble transferrin receptor,s Tf R.)在地中海贫血(地贫)合并缺铁诊断中的实际应用价值。方法对19例静止型α地贫,51例标准型α地贫,81例β地贫杂合子,59例缺铁性贫血,30例地贫合并缺铁及50例正常健康儿童的血红蛋白(Hb)含量,血清铁蛋白(serum ferritin,SF.)水平以及s Tf R水平进行检测,并对结果进行统计学分析。结果地贫患儿的s Tf R水平与正常对照相比无显著性差异或仅略高于正常对照,单纯缺铁性贫血患儿的s Tf R水平高于正常对照,地贫合并缺铁患儿的s Tf R水平明显高于单纯地贫患儿。结论 s Tf R可作为一项反映地贫合并缺铁患儿机体铁状况的指标,用于对此类患儿的诊断及铁剂治疗后的疗效监测。Objective：To evaluate the usefulness value of analysis of serum soluble transferrin receptor（sTfR）in diagnosis of children with concomitant thalassemia trait and iron deficiency anemia. Methods：240 blood samples from 19 children with silent α thalassemia,51 children with standard α thalassemia,81 children with β thalassemia trait,59 children with iron deficiency anemia,30 children with concurrent thalassemia and iron deficiency and 50 normal individuals were detected for the values of hemoglobin（Hb）,serum ferritin（SF）and sTfR,and the results were statistically analyzed. Results：There was no distinct difference between the children with silent α thalassemia and normal controls on the values of sTfR. The values of sTfR in children with standard α thalassemia and β thalassemia trait were slightly higher than those of normal controls. The values of sTfR in children with iron deficiency were higher than those of normal controls. The values of sTfR in children with concurrent thalassemia and iron deficiency were significantly higher than those of other groups. Conclusion：The sTfR level could be used as a determinant of iron condition in the body of the children with concurrent thalassemia and iron deficiency,and an index of clinical treatment effects in these children.
