超声乳化人工晶体植入联合小梁切除术治疗原发性闭角型青光眼合并白内障疗效及安全性分析

摘　　要：目的探讨超声乳化人工晶体植入联合小梁切除术治疗原发性闭角型青光眼合并白内障疗效及安全性分析。方法选取2015年10月～2017年10月本院收治的PACG-C患者86例,依据随机数字表法分为超乳组和常规组,每组43例,常规组给予单纯小梁切除术治疗,超乳组给予超声乳化人工晶体植入联合小梁切除术治疗,比较两组滤过泡、并发症、视力、眼压、角膜散光度。结果超乳组和常规组功能性滤过泡形成率、并发症发生率基本相同,差异无统计学意义(P> 0.05);超乳组和常规组术后1个月视力明显高于术前,超乳组术后1个月视力明显高于常规组,超乳组和常规组术后1个月眼压明显低于术前,超乳组术后1个月眼压明显低于常规组,差异有统计学意义(P <0.05);常规组术后1个月角膜散光度明显高于术前,差异有统计学意义(P <0.05),超乳组术前术后1个月角膜散光度基本相同,差异无统计学意义(P> 0.05)。结论超声乳化人工晶体植入联合小梁切除术治疗可有效改善PACG-C患者的视力、眼压,对患者角膜散光度影响少,且具有良好的安全性,值得临床作进一步推广。Objective To evaluate the efficacy and safety of phacoemulsifieation and intraoeular lens implantation combined with trabeeuleetomy in the treatment of primary angle-closure glaucoma with cataract （PACG-C）. Methods Eighty-six cases of PACG-C treated in our hospital from October 2015 to October 2017 were selected. According to the random number table method, the patients were divided into the super emulsion group and the conventional group, 43 cases in each group. The conventional group was treated with simple trabeculectomy. The super emulsion group was treated with phacoemulsification and intraoeular lens implantation combined with trabeculectomy. The blebs, complications, visual acuity, intraocular pressure and corneal astigmatism were compared between the two groups. Results There was no significant difference in the formation rate of functional bleb and the incidence of complications between the two groups （P 〉 0.05）. The visual acuity of the super emulsion group and the conventional group was significantly higher than that of the preoperative 1 months after operation. The visual acuity of the super emulsion group was significantly higher than that of the conventional group one month after operation, and the intraoeular pressure of the super emulsion group and the conventional group one month after operation was significantly lower than that before operation. The intraoeular pressure of the super emulsion group was significantly lower than that of the conventional group at 1 months after operation （P 〈 0.05）. The corneal astigmatism of the conventional group was significantly higher than that of the preoperative 1 months （P 〈 0.05）. Corneal astigmatism was basically the same in 1 months before and after operation, and the difference was not statistically significant （P 〉 0.05）. Conclusion Phacoemulsification and intraocular lens implantation combined with trabeculectomy can effectively improve the visual acuity and intraoeular pressure of patients with PACC-C. It has little effect
