摘  要:目的:探讨首发精神分裂症患者前注意加工功能与其血清同型半胱氨酸(Hcy)水平之间的关系。方法:采用高效液相色谱法测定36例首发精神分裂症患者和38例健康对照组的空腹血清Hcy水平,采用32导事件相关电位(ERP)测试仪测定患者组和对照组失匹配负波(MMN)潜伏期和波幅值,并将患者组MMN潜伏期、波幅值与血清Hcy浓度进行相关分析。结果:患者组Fz、Cz、Pz处MMN潜伏期较对照组延长,但差异无统计学意义;患者组Fz、Cz、Pz处MMN波幅较对照组降低,血清Hcy浓度较对照组明显增高,差异有统计学意义(t=-18.763,-15.232,-7.821,9.396;P〈0.05);患者组MMN波幅值Pz〉Cz〉Fz(t=6.172,3.550;P〈0.05),对照组MMN波幅值Fz〉Cz〉Pz(t=3.542,3.386;P〈0.05),对照组脑地形图MMN波幅能量分布以前额、中央区为主,而患者组脑地形图MMN波幅能量分布以中央、顶区为主,患者组脑地形图MMN波幅能量分布较对照组主效应后移。相关分析得出Fz、Cz、Pz处MMN平均潜伏期与血清Hcy水平呈正相关(t=0.423,P〈0.05),Fz、Cz、Pz处MMN平均波幅与血清Hcy水平呈负相关(t=-0.547,P〈0.05)。结论:首发精神分裂症患者存在前注意自动加工功能损害,且与其血清Hcy水平升高存在一定的相关性。Objective: To explore the relationship between the pre-attention functions and serum homocysteine (Hcy)levels of first-episode schizophrenia patients.Methods: The levels of serum Hcy in 36 first--episode schizophrenia patients and 38 cases of healthy control group were measured by reversephase high performance liquid chromatography.The values of latencies and amplitudes of mismatch negative wave(MMN)were measured by 32 event related potential.Correlation analysis was done between MMN latencies, amplitudes and serum Hcy level. Resuits:The MMN latencies of Fz,Cz and Pz were not longer in the patient group than that in the control group;The M MN amplitudes of Fz, Cz and Pz were lower in the patient group than that in the control group (t =-18. 763, -15.232,-7.821;P〈0.05), and the serum H cy level was much higher in the patient group than that in the control group(t = 9.396, P〈0.05).The values of MMN amplitudes of the patient group were Pz〉Cz〉Fz(t = 6.172,3.550; P 〈0.05), the values of MMN amplitudes of the control group were Fz〉 Cz〉 Pz ( t = 3. 542,3. 386 ; P (0.05). The brain--topographic map energy of the MMN amplitudes of the control group were given priority to forehead and central area and given priority to central and top area in the patient group.The main effect of MMN amplitudes energy distribution of the patient group moved more backward than that of the control group, but MMN latencies energy distribution were no difference in each region.Correlation analysis showed MMN average latencies of Fz,Cz and Pz had positive correlations with serum Hey Levels(t = 0.423, P〈0.05), MMN average amplitudes of Fz, Cz and Pz had negative correlations with serum Hey Levels (t =-0.547, P 〈 0.05 ).Conclusion: The first- episode schizophrenia patients have pre-attention automatic processing damages and there are certain correlations between the damages and the rise of serum Hcy levels.
