高频多普勒超声检查联合血清肿瘤标志物对卵巢良、恶性肿瘤诊断价值研究

摘　　要：目的研究分析高频多普勒超声检查联合血清肿瘤标志物对卵巢良、恶性肿瘤诊断价值。方法选取2014年6月—2016年12月就诊的56例卵巢恶性肿瘤患者,56例卵巢良性肿瘤患者,56例健康者作为研究对象。采用回顾性分析法,比较所有研究者高频多普勒超声检查结果、血清糖类抗原125(CA125)、血清人附睾蛋白4(HE4),并比较上述水平间的差异,记录不同指标在研究中的阳性表达及不同检测方法对卵巢肿瘤的灵敏度和特异性。结果通过分析卵巢肿瘤恶性组血清CA125、HE4水平高于卵巢良性肿瘤组和健康组,3组上述指标比较差异有统计学意义(P<0.05);卵巢恶性肿瘤组与良性肿瘤组比较,CA125与超声诊断单项检测的阳性率差别有统计学意义(P<0.05),3项单独检测以及3项中两两联合检测卵巢肿瘤的阳性率均低于3项联合检测,3项联合检测卵巢恶性肿瘤组阳性率高于卵巢良性肿瘤组,差异有统计学意义(P<0.05);在联合检测中,CA125与HE4联合超声诊断卵巢良恶性肿瘤的灵敏度、特异度均相对较高,比较差异具有统计学意义(P<0.05)。结论运用高频多普勒超声检查联合血清CA125以及HE4对诊断卵巢肿瘤性质有诊断价值,有助于提高诊断准确性,改善患者预后。ObjectiveTo study the diagnostic value of high frequency Doppler ultrasonography combined with serum tumor markers in benign and malignant ovarian tumors. MethodsFifty-six patients with malignant ovarian tumors, 56 patients with benign ovarian tumors and another 56 healthy subjects were enrolled in this study between June 2014 and December 2016. The levels of serum carbohydrate antigen 125（CA125）and serum human epididymal protein 4（HE4）were compared with the outcomes of high frequency Doppler ultrasonography, and the difference between the above levels was recorded using retrospective analysis. The sensitivity and specificity of different detection methods to ovarian tumors were recorded. ResultsThe levels of serum CA125 and HE4 in the malignant group were significantly higher than those in the benign ovarian tumor group and healthy group（P〈0.05）. The difference in the positive rate of CA125 and ultrasonic diagnosis was statistically significant between the malignant group and the benign group（P〈0.05）. The positive rates of the three individual tests and two combined tests for ovarian tumors were lower than those of the three combined tests. The positive rate of the malignant group was higher than that of the benign group（P〈0.05）. In combined detection, the sensitivity and specificity of CA125 and HE4 combined with ultrasound in the diagnosis of ovarian benign and malignant tumors were relatively high, and the difference was statistically significant（P〈0.05）. ConclusionHigh frequency Doppler ultrasonography combined with serum CA125 and HE4 can be used to diagnose ovarian cancer, which can improve the diagnostic accuracy and improve the prognosis of patients.
