微血管减压术下原发性三叉神经痛患者预后影响因素分析

摘　　要：目的探讨影响原发性三叉神经痛患者行微血管减压术的预后因素。方法回顾性分析2015年3月至2017年3月在本院实施微血管减压术的60例原发性三叉神经痛患者的临床资料,单因素分析可能影响术后疗效的相关指标,对单因素分析有意义的指标进行多因素Logistic回归分析。结果手术后痊愈43例,有效15例,无效2例,有效率96. 67%。手术后出现伤口炎症2例,耳鸣1例,口角疱疹1例,发热2例,头痛、恶心3例,并发症发生率15%。无死亡病例,亦无出现面瘫、出血、脑梗塞等并发症。经过单因素分析,年龄、患侧、性别与预后无明显关系,差异无统计学意义(P> 0. 05),病程、减压程度、临床分型、压迫程度与预后有直接关系,差异有统计学意义(P <0. 05)。经过多因素Logistic回归分析,病程、临床分型、压迫程度、减压程度是影响预后的独立危险因素(P <0. 05)。结论病程、临床分型、压迫程度、减压程度是影响微血管减压术治疗原发性三叉神经痛患者预后的因素。Objective To investigate the prognostic factors of microvascular decompression in patients with primary trigeminal neuralgia. Methods The data of 60 patients with primary trigeminal neuralgia who underwent microvascular decompression in our hospital from March 2015 to March 2017 were retrospectively analyzed. The univariate analysis may affect the relevant indicators of postoperative efficacy. Analyze meaningful indicators for multivariate logistic regression analysis. Results After operation,43 cases were cured,15 cases were effective,2 cases were ineffective,and the effective rate was 96. 67%. After surgery,there were 2 cases of wound inflammation,1 case of tinnitus,1 case of oral herpes,2 cases of fever,3 cases of headache and nausea,and 15% of complication rate.No deaths occurred,and no complications such as facial paralysis,hemorrhage,and cerebral infarction occurred. After univariate analysis,there was no significant relationship between age,affected side,gender,and prognosis. The difference was not statistically significant（ P 〉 0. 05）. The course of disease,degree of decompression,clinical classification,and degree of oppression were directly related to prognosis. The differences were statistically significant（ P 〈 0. 05）. After multivariate Logistic regression analysis,there was a certain correlation between disease course,clinical classification,degree of compression,degree of decompression,and prognosis（ P 〈 0. 05）.Conclusion The course of disease,clinical classification,degree of oppression,and degree of decompression are prognostic factors affecting microvascular decompression in patients with primary trigeminal neuralgia.
