青少年长期佩戴角膜塑形镜治疗近视的疗效观察

摘　　要：目的探讨角膜塑形镜对青少年近视患者视力、角膜形态变化及泪液的影响。方法随机选取配戴角膜塑形镜的青少年近视患者44例(88眼),观察戴镜前和戴镜后1w、1mon、3mon、6mon、1a等效球镜、裸眼视力、矫正视力、角膜曲率、泪液基础分泌量、泪膜破裂时间,对比分析检查结果。结果配戴角膜塑形镜后1a与戴镜前相比,屈光度明显降低,平均等效球镜从-3.05±0.79下降到-1.32±0.42,P<0.05;UCVA从0.14±0.07升高到1.01±0.04,P<0.05;BCVA戴镜前0.97±0.05,戴镜后0.98±0.06,P>0.05;戴镜前角膜曲率43.65±1.07D,戴镜后1a为42.15±1.03D,P<0.05;泪液基础分泌量戴镜前后各时间点比较,差异均无统计学意义(P>0.05);戴镜后BUT为10.44±2.52,与戴镜前7.38±2.73比较差异具有统计学意义(P<0.05)。结论角膜塑形镜配能有效降低近视度,显著提高裸眼视力,不影响角膜曲率及泪液的基础分泌,但泪膜稳定性下降。Objective To investigate the effect of orthokeratology on vision and corneal shape change and tears of adolescents myopia. Methods 44 cases（88 eyes） myopia young patients with orthokeratology were randomly selected,The equivalent spherical mirrors, uncorrected visual acuity（UCVA）, corrected visual acuity（ BCVA）, corneal curvature, tear secretion and tears break up time（BUT） were examined before wearing glasses, wearing glasses after 1 week,1 month, 3 months, 6 months and 1 year, the test results were compared and analyzed. Results Diopter decreased obviously after wearing orthokeratology for 1 year, The average equivalent spherical mirrors decreased from 一 3. 05 ±0.79 to-1. 32±0. 42, the difference has statistics significant compared with before; UCVA increased from 0. 14±0. 07 to 1. 01 ±0.04,the difference has statistics significant compared with before; Before wearing orthokeratology BCVA was 0. 97±0. 05,and after wearing glasses BCVA was 0. 98±0. 06,the differences has no statistical significance, there was no significant statistics difference. There was no significant statistics difference of tear secretion before and after wearing orthokeratology; After wearing orthokeratology BUT was 10. 44 ±2. 52, before wearing orthokeratology BUT was7. 38 ±2. 73, the difference has statistics significant. Conclusions Orthokeratology could effectively reduce the degree of myopia,significantly improve uncorrected visual acuity. Wearing orthokeratology does not affect the corneal curvature and tear secretion, but it affect the stability of tear.
