摘 要:目的探讨两种不同方式的上斜肌减弱术对A型斜视的影响。方法对67例伴上斜肌亢进的A型斜视患者施行上斜肌断腱术及上斜肌延长术,比较术前术后A征矫正情况、上斜肌功能状况、客观旋转斜度及双眼视觉等方面。结果上斜肌断腱术A征矫正量27.29~△±14.82~△,上斜肌延长术A征矫正量23.91~△±9.08~△,上斜肌断腱术术后与术前相比总内旋转减少11.27°±5.81°,上斜肌延长术总内旋转减少11.65°±3.99°,与术前相比差异有统计学意义(P<0.01),两者之间相比差异无统计学意义(P>0.05)。结论上斜肌断腱术及上斜肌延长术均是治疗伴上斜肌亢进的A型斜视的有效术式。Objective To evaluate the efficacy of two kinds of superior oblique weakening to A-pattern strabismus. Methods A total of 67 patients with the superior oblique overaction and A-pattern strabismus were treated with superior oblique tenectomy and superior oblique tendon-expander. A-pattern strabismus, function of superior oblique, fovea-disc angle( FDA) and binocular visual function were examined pre-and post-operatively. Results The mean A-pattern strabismus degrees 27. 29~△ ±14. 82~△ was corrected by superior oblique tenectomy and 23. 91~△ ±9. 08~△ was corrected by tendonexpander. The mean correction of FDA was 11. 270±5. 810 corrected by superior oblique tenectomy and 11. 65°±3. 990 was corrected by tendon-expander. The comparisons of A-pattern strabismus degrees and FDA showed high significant differences(P〈0. 01) pre-and post-operatively. There were no significant differences( P〉0. 05) between the effects of superior oblique tenectomy and tendon-expander. Conclusions Superior oblique tenectomy and tendon-expander are effective approach to A-pattern strabismus with the superior oblique overaction.
