斜视矫正术对眼压影响的机制探讨

摘　　要：目的测量三种斜视矫正手术术前、术中、术后眼压的变化,探讨斜视手术对眼压的影响机制。方法选取我院进行斜视矫正手术的54例共同性外斜视患者(102眼),按手术方式分为外直肌后退组、内直肌缩短组、外直肌后退联合内直肌缩短组,各组在术前1天、术前5分钟、术后5分钟、术后1天、术后1周眼压测量,分析对比不同手术方式后眼压的变化。结果各组患者术后5分钟眼压均有明显下降,眼压下降程度以外直肌后退联合内直肌缩短组最大,其次为内直肌缩短组,再次为外直肌后退组。外直肌后退组及内直肌缩短组眼压术后1天眼压回升至术前水平,外直肌后退联合内直肌缩短组眼压术后1天仍低于术前,各组术后1周眼压均较术前升高。结论共同性外斜视矫正术可引起眼压波动,与手术方式相关,考虑与不同手术方式所离断的眼外肌中睫状血管分布有关。Objective To investigate the mechanism of the effect of strabismus surgery on intraocular pressure（IOP） through the measurement of IOP pre and post operation. Methods 102 eyes of 54 concomitant strabismus patients who had undergone surgery for correction of exotropia were grouped according to the types of operation ： lateral rectus recession group, internal rectus resection group, lateral rectus recession-internal rectus resection group. The data were selected from the measurement of IOP in 1 day, 5 min before operation and 5 min, 1 day, 7 day after operation in each group. Result IOP in 5 min post operation were significant decreased in all of three groups, the largest decline group was the lateral rectus recession-internal rectus resection group, the second one was the internal rectus resection group, the last one was the lateral rectus recession group. The IOP were recovered in internal rectus resection group and lateral rectus recession group at 1 day after operation. IOP in the lateral rectus recession-internal rectus resection group were still below the level pre-operation. IOP in lweek after operation were increased than pre-operation in these groups. Conclusion Horizontal rectus muscle detachment can caused intraocular pressure fluctuation, the effect were related to the distribution of anterior ciliary vascular.
