摘　　要：目的:评估抗病毒治疗对血清AFP在HBV感染相关肝癌诊断应用中的影响。方法:回顾性收集并分析365例慢性乙型肝炎、858例慢性乙肝肝硬化和663例HBV感染相关肝细胞癌患者的临床资料和抗病毒治疗情况。结果:接受抗病毒治疗的慢性乙型肝炎、肝硬化和HCC患者血清AFP水平均低于未治疗组。血清AFP诊断HCC的ROC曲线分析结果显示,诊断界值为400μg/L时,治疗组灵敏度为19.6%,未治疗组特异度为97.0%;在特异度为97.0%的水平上,治疗组的诊断界值为100μg/L,此时灵敏度为29.3%。结论:抗病毒治疗会影响血清AFP对HBV感染相关HCC的诊断。Aim： To evaluate the influence of antiviral therapy on the diagnosis value of AFP for HBV-related hepatocellular carcinoma（ HCC）. Methods：Clinical data and antiviral treatment information of 365 cases of chronic hepatitis B（CHB）,858 cases of chronic hepatitis B cirrhosis and 663 cases of HBV-related HCC were retrospectively collected. Results：The serum AFP level of the patients accepting the antiviral treatment（ the treated group） was lower than that in the untreated patients（ the untreated group）. The ROC of serum AFP level for HCC diagnosis showed that the sensitivity was19. 6% in the treated group and the specificity was 97. 0% in the untreated group at the boundary of 400 μg/L;To keep the specificity at 97. 0%,the boundary was 100 μg/L in the treated group,the sensitivity was 29. 3%. Conclusion： Antiviral therapy could affect the diagnosis of serum AFP level for HCC.
