开窗减压术治疗颌骨大型囊性肿瘤临床初步研究

摘　　要：目的:探讨开窗减压术治疗上、下颌骨大型囊性肿瘤的效果以及应用价值,旨在为该疗法的临床应用提供可靠依据。方法:前瞻性随机选取2015年5月-2016年5月笔者医院收治的108例大型囊性肿瘤患者,采取随机数字表法将其分为两组。观察组:54例,采用开窗减压术;对照组:54例,采取传统囊肿刮除术,随访调查美观度影响,记录患者病变大小形态的改变、下唇感觉功能检测、受累牙附近牙牙髓活力检测、感染发生率及复发发生率。结果:两组患者囊腔缩小情况显示,观察组54例患者美观度优良率、牙髓活力检测以及病变侧下唇感觉评分明显优于对照组,差异有统计学意义(P<0.05);观察组患者未出现病理性骨折、复发等情况,复发率明显低于对照组(P<0.05)。结论:大型囊性肿瘤患者行开窗减压术疗效较佳,有较高安全性,并且可恢复美观度,值得临床推广应用。Objective To investigate the effect of window decompression on the treatment of large cystic tumors of upper and lower jaw and its application value. The aim is to provide a reliable basis for the clinical application of this therapy. Methods A total of 108 patients with large cystic tumors were randomly selected from May 2015 to May 2016 in our hospital. 108 patients were divided into two groups using a random number table method. In the observation group, 54 patients underwent decompression of the fenestration, and 54 patients in the control group underwent conventional cystectomy. Changes in the patient＇s lesion size and morphology, detection of lower lip sensory function, detection of dental pulp and pulp vitality near the affected tooth, incidence of infection, and incidence of recurrence were recorded. Results The cystic shrinkage of the two groups showed that the excellent rate of aesthetics, the detection of pulp vitality and the sensory score of the lower lip of the lesion were signifcantly better in the observation group than in the control group （P〈0.05）. There were no pathological fractures and recurrences in the observation group, and the recurrence rate was signifcantly lower than that in the control group （P〈0.05）. Conclusion Patients with large cystic tumors have better curative effect, higher safety, and can restore aesthetics. It is worthy of clinical promotion and application.
