“胸痛优先”分诊方法对胸痛患者诊治及预后的影响

摘　　要：目的:探讨"胸痛优先"分诊方法对胸痛患者诊治及预后的影响。方法:设计胸痛患者分诊记录单,实时录入2015年1月1日—2017年12月31日急诊科接诊的胸痛患者临床资料,根据"胸痛中心(CPC)"成立时间,分成对照组和观察组,对照组沿用常规方法进行分诊,观察组采用"胸痛优先"分诊方法进行分诊,比较两组患者的分诊准确率、患者满意度、急诊诊疗时间、1周内死亡率及3个月内死亡率的差异。结果:观察组406例,对照组384例;观察组分诊准确率及患者满意度均明显高于对照组(P <0. 05);观察组急诊诊疗时间、1周内死亡率及3个月内死亡率均显著低于对照组(P <0. 05)。结论:"胸痛优先"分诊方法可明显提高胸痛患者的分诊准确率,缩短急诊诊疗时间,改善患者近期预后,提高患者对医护工作的满意度。Objective ：To discuss the influence of pectoralgia - priority triage method on clinic procedures and prognosis of patients with chest pain. Methods ： To design triage registration form of patients with chest pain to register timely the clinical data of patients with chest pain from emergency department during January 2015 to December 2017. The patients were divided into control group and experimental group on the basis of the time that ＂chest pain center （CPC） ＂ in hospital was established. The patients from control group were used common triage method and those from experimental group were adopted peetoralgia - priority triage method. The accuracy rate of triage, the degree of patients satisfieing, the clinic procedure time that patients were treated in emergency department, mortality in a week and mortality in there months of two groups were compared. Results： All 406 patients were enrolled into experimental group and 384 patients were enrolled into control group. The accuracy of diagnosis and patient satisfaction were significantly higher than those of the control group （ P 〈 0.05 ）. The time of emergency treatment, mortality within 1 week and mortality within 3 months of the observation group were significantly lower than those of the control group （ P 〈 0.05 ）. Conclusion ： The peetoralgia - priority- triage method can increase obviously the accuracy rate of tri- age of patients with chest pain, shorten the clinic procedure time that patients were treated in emergency department, improve patientsiorognosis and enhance the degree of patients satisfieing for medical and care work in our hospital.
