摘 要:目的探讨轴性高度近视眼患者白内障超声乳化吸除联合人工晶体植入术后屈光度数误差原因,对比SRK/T公式和Haigis公式预测屈光度数的准确度。方法方便选取2015年1月—2018年4月在该院接受白内障超声乳化吸除联合人工晶体植入术的轴性高度近视眼患者92例,分析屈光度数误差的分布特征及各个参数与屈光度数误差的相关性,比较SRK/T公式和优化Haigis公式的准确度。结果不同眼轴长度、前房深度、角膜曲率、角膜横径患者屈光度数误差差异有统计学意义(t=2.330,1.993,2.104,2.046;P=0.020,0.047,0.037,0.042)。眼轴长度(r=0.782,P=0.012)、角膜曲率(r=0.720,P=0.035)与轴性高度近视眼患者白内障超声乳化吸除联合人工晶体植入术后屈光度数误差呈明显正相关关系。SRK/T、Haigis公式的屈光度数误差绝对值分别为(0.42±0.17)D、(0.73±0.24)D,差异有统计学意义(t=2.479;P=0.014)。结论眼轴长度>30 mm、角膜曲率>44 D是引起轴性高度近视眼患者白内障超声乳化吸除联合人工晶体植入术后屈光度数误差的主要原因,与Haigis公式相比,SRK/T公式预测屈光度数的准确度更高。Objective This paper tries to investigate the causes of diopter error after phacoemulsification and intraocular lens implantation in patients with axial high myopia, and to compare the accuracy of calibrated power with SRK/T formula and Haigis formula. Methods A total of 92 patients with axial high myopia who underwent phacoemulsification and intraocular lens implantation in the hospital from January 2015 to April 2018 were convenient selected. The distribution characteristics of diopter error and the errors of diopter error were analyzed. The correlation is compared with the accuracy of the SRK/T formula and the optimized Haigis formula. Results There were significant differences in the refractive error of patients with different axial length, anterior chamber depth, corneal curvature, and corneal transverse diameter(t=2.330, 1.993, 2.104, 2.046; P=0.020, 0.047, 0.037, 0.042). The length of the axial length (r=0.782, P=0.012), corneal curvature (r=0.720, P=0.035) was significantly positively correlated with the diopter error after phacoemulsification and intraocular lens implantation in patients with axial high myopia. The absolute values of the diopter error of the SRK/T and Haigis formulas were (0.42±0.17)D and (0.73±0.24)D, respectively, and the difference was statistically significant (t=2.479; P=0.014). Conclusion The axial length 〉30 mm and corneal curvature〉44 D are the main reasons for the diopter error after phacoemulsification and intraocular lens implantation in patients with axial high myopia. Compared with the Haigis formula, the SRK/T formula predicts the diopter. The accuracy of the number is higher.
