补肾活血汤联合康柏西普治疗渗出型年龄相关性黄斑变性临床观察

摘　　要：目的:观察补肾活血汤联合康柏西普治疗渗出型年龄相关性黄斑变性(AMD)肝肾阴虚兼血瘀证的临床疗效。方法:将58例渗出型AMD肝肾阴虚兼血瘀证患者随机分为对照组29例(31眼)和治疗组29例(30眼),对照组予康柏西普眼用注射液注射玻璃体,治疗组在对照组治疗的基础上加用补肾活血汤内服,2组均连续治疗12周。分别于治疗前,治疗4周、8周和12周采用国际标准视力表检查裸眼视力,观察黄斑中心凹视网膜厚度(CMT)、黄斑部荧光渗漏及遮蔽荧光的变化情况,评定中医证候评分。结果:治疗4、8、12周,2组的视力均得到逐渐的改善,CMT均较治疗前逐渐降低,差异均有统计学意义(P <0.05);治疗组治疗8周和12周的视力均优于对照组,CMT均小于对照组,差异均有统计学意义(P <0.05)。2组中医证候评分均低于治疗前(P <0.01),治疗组评分低于对照组(P <0.01)。治疗组眼底荧光造影术(FFA)疗效与视力疗效均优于对照组(P <0.05)。结论:补肾活血汤内服联合玻璃体注射康柏西普治疗渗出型AMD肝肾阴虚兼血瘀证患者,在提高视力、缩小CMT、缩小黄斑区病灶范围、改善症状等方面均优于单纯使用康柏西普治疗,值得在临床使用。Objective： To observe the clinical effect of Bushen Huoxue tang combined with conbercept for age-related macular degeneration（AMD） of exudative type with the syndrome of liver-kidney yin deficiency and blood stasis. Methods：Divided 58 cases of exudative type AMD patients with deficiency syndrome of both liver and kidney yin together with blood stasis into the control group（31 eyes） and the treatment group（30 eyes） randomly,29 cases in each group. The control group was given conbercept ophthalmic injection of the vitreous body,while the treatment group was additionally given the oral administration of Bushen Huoxue tang. The treatment of both groups consecutively lasted for 12 weeks. Examined the uncorrected visual acuity before the treatment as well as 4,8 and 12 weeks after the treatment respectively by using the international standard visual acuity chart. Detected the central macular thickness（CMT） of the macular,observed the changes of fluorescence leakage of the macular area and the blocked fluorescence, and evaluated the score of chinese medicine syndrome. Results：At 4 th,8 th and 12 th week of treatment,the visual acuity of both groups gradually improved,and the CMTs were decreased gradually when compared with those before treatment, differences being significant（P〈0.05）. The visual acuity in the treatment group at the 8 th and 12 th week were better than those in the control group,while the CMTs were smaller than those in the control group,differences being significant（P〈0.05）. The scores of chinese medicine syndrome in both groups were lower than those before and after treatment（P〈0.01）. The scores in the treatment group were lower than those in the control group（P〈0.01）.The curative effect of FFA and visual acuity in the treatment group were better than those in the control group（P〈0.05）.
