摘 要:目的分析医疗从事放射工作人员(简称从放人员)外周血淋巴细胞畸变状况及医疗非从放人员的淋巴细胞畸变本底值。方法对549名医疗从放人员及525名医疗非从放人员的外周血淋巴细胞畸变检测数据进行统计分析,比较不同性别、不同工龄、不同岗位医疗人员外周血淋巴细胞微核数目均值及染色体畸数目均值差异。结果医疗从放组人员外周血淋巴细胞微核数目均值为(1.90±1.62)个,非从放组人员淋巴细胞微核数目均值为(0.53±1.11)个,二者差异有统计学意义(t=16.204,P<0.05)。医疗从放组人员外周血淋巴细胞染色体畸变数目均值为(0.15±0.50)个,非从放组人员染色体畸变数目均值为(0.07±0.36)个,二者差异有统计学意义(t=2.982,P<0.05)。医疗从放人员0~9年工龄组微核数目均值为(1.53±1.55)个,低于其他工龄组,差异有统计学意义(F=2.791,P<0.05);医疗从放人员核医学组与介入治疗组微核数目均值分别为(2.50±1.60)、(2.59±1.75)个,高于其他岗位组,差异有统计学意义(F=2.850,P<0.05);医疗从放人员介入从放组染色体畸变数目均值为(0.35±0.84)个,高于诊断从放组、牙科从放组及放疗组,差异有统计学意义(F=4.263,P<0.05);医疗非从放人员10~19年工龄组微核数目均值为(0.56±1.15)个,低于30年以上工龄组(0.89±1.37)个,差异有统计学意义(F=0.333,P<0.05)。结论长期小剂量接触电离辐射后可造成医疗从放人员细胞遗传学损伤,工龄长者及核医学与介入治疗岗位者损伤程度较大。需进一步加强医疗从放人员职业健康保护。Objective To analyze the changes of peripheral blood lymphocyte aberration in medical radiation workers and the background value of peripheral blood lymphocyte aberration in medical non-radiation workers. Methods 549 cases of medical radiation workers and 525 cases of non-radiatian medical workers' peripheral blood lymphocyte detection datas were statistically analyzed,and the peripheral blood lymphocyte micronucleus average number and the chromosome aberration average number were compared among different genders,different ages and different positions. Results The average number of peripheral blood lymphocytes micronucleus in medical radiation group was( 1. 90 ± 1. 62),and the average number of lymphocyte micronucleus was( 0. 53 ± 1. 11) in non-randomized groups. The differences between the two groups were statistically significant( t = 16. 204,P〈0. 05). The average number of peripheral blood lymphocytes chromosome aberration in medical radiation group was( 0. 15 ± 0. 50),and it was( 0. 07 ± 0. 36) in medical non-radiation group. The differences between the two groups were statistically significant( t = 2. 982,P〈0. 05). The average number of micronucleus in medical radiation workers of 0 - 9 working year group was( 1. 53 ± 1. 55),which was lower than that in the other working year groups,and the differences were statistically significant( F = 2. 791,P〈0. 05). The average number of micronucleus in nuclear medicine group and intervention group were( 2. 50 ± 1. 60) and( 2. 59 ± 1. 75) respectively,which were higher than that in the other post groups,and the differences were statistically significant( F = 2. 850,P〈0. 05). The average number of chromosome aberration in interventional group was( 0. 35 ± 0. 84),which was higher than that in diagnostic radiation groups,dental radiation groups and radiotherapy group,and the differences were statistically significant( F = 4. 263,P〈0. 05).
