海马保护性全脑放疗对脑转移瘤患者认知功能的影响

摘　　要：目的探讨海马保护性全脑放疗对脑转移瘤患者认知功能的影响。方法将60例脑转移瘤患者随机分为研究组和对照组,各30例。研究组行海马保护性全脑放疗,对照组行常规全脑放疗。在放疗前1周、放疗后6个月采用蒙特利尔认知评估(MoCA)量表及简易精神状态(MMSE)量表评估患者的认知功能,对比两组患者认知功能障碍的发生情况。结果放疗后6个月,两组患者的MoCA、MMSE认知评分均较放疗前下降,研究组的MoCA、MMSE认知评分高于对照组,且研究组MoCA、MMSE认知评分下降幅度低于对照组(均P <0. 05);采用MoCA或MMSE进行认知功能评价时,研究组认知功能障碍发生率均低于对照组(均P <0. 05)。结论相对于常规全脑放疗,海马保护性全脑放疗对脑转移瘤患者认知功能有一定的保护作用。Objective To investigate the influence of whole brain radiotherapy with hippocampus protection on the cognitive function of patients with brain metastatic encephaloma. Methods Sixty patients with metastatic encephaloma were randomly divided into study group and control group,30 cases in each group.The study group received whole brain radiotherapy with hippocampus protection,while the control group received conventional whole brain radiotherapy.One week before and six months after radiotherapy,the Montreal Cognitive Assessment Scale（MoCA） and the Mini-Mental Status Examination（MMSE） were used to assess the cognitive function of the patients,and the incidence of cognitive dysfunction was compared between the two groups. Results Six months after radiotherapy,the MoCA and MMSE scores for cognitive function decreased in both groups compared with those before radiotherapy,the MoCA and MMSE scores for cognitive function in the study group were higher than those in the control group,and the decrease of the MoCA and MMSE scores for cognitive function in the study group was lower than that in the control group（all P 〈0.05）;the incidence rate of cognitive dysfunction in the study group was lower than that in the control group using either MoCA or MMSE for the cognitive function assessment（all P 〈0.05）. Conclusion Compared with conventional whole brain radiotherapy,whole brain radiotherapy with hippocampus protection has a certain protective effect on the cognitive function of patients with metastatic encephaloma.
