胆胰管内支架植入联合胆宁片治疗胆源性急性复发性胰腺炎的效果

摘　　要：目的探讨内镜下乳头肌切开(EST)取石术后胆胰管内支架植入联合胆宁片治疗胆源性急性复发性胰腺炎(ARP)的疗效。方法回顾性分析65例胆源性ARP患者的临床资料。对照组患者(n=33) EST取石术后清理胆道,行胆、胰管内支架植入,观察组患者(n=32)在此基础上给予口服胆宁片。治疗后随访3年,比较两组患者ARP再次发生率。结果观察组EST术后第1年、第2年分别有2例、1例ARP再次发生,对照组EST术后第1年、第2年、第3年分别有9例、1例、1例ARP再次发生,观察组ARP再次发生率低于对照组(P <0. 05)。结论胆源性ARP患者行EST取石术后给予胆胰管支架植入联合胆宁片治疗可预防ARP的复发,使患者长期获益。Objective To explore the efficacy of biliary and pancreatic duct stenting combined Danning Tablets on biliary acute recurrent pancreatitis（ARP） after endoscopic sphincterotomy（EST）. Methods The clinical data of 65 cases of biliary ABP were retrospectively analyzed.Patients in the observation group（ n =33） received biliary tract cleaning after EST,and then underwent biliary and pancreatic duct stenting,while patients in the control group（ n =32） were given Danning Tablets orally based on the therapy performed in the control group.After a 3-year post-treatment follow-up,the recurrence rates of ARP were compared between the two groups. Results In the observation group,2 and 1 case suffered from ARP recurrence in the first and second year after EST respectively,in observation group,9,1 and 1 case suffered from ARP recurrence in the first,second and third year respectively,the recurrence rate of ARP in the observation group was lower than that in the control group（ P 〈0.05）. Conclusion The application of biliary and pancreatic duct stenting combined with Danning Tablets after EST can prevent the patients with biliary ARP from the recurrence of ARP,and the patients might obtain long-term benefits.
