保乳术与改良根治术治疗早期乳腺癌疗效及生存质量分析

摘　　要：目的分析保乳手术与改良根治术对早期乳腺癌疗效及生存质量的影响.方法 60例早期乳腺癌患者根据手术方式分为改良组与保乳组,观察比较两组患者的术中情况、近期疗效、美学效果、术后并发症及生存质量.结果保乳组的手术时间、术中出血量、住院时间较改良组明显降低(P<0.05);保乳组及改良组美容效果的优良率分别为93.3%(28/30),60%(18/30),结果显示保乳组的美容效果具有优越性(P<0.05);保乳组及改良组术后并发症发生率分别为13.2%,43%,差异具有统计学意义(P<0.05);两组患者术后1 a远处转移率、肿瘤局部复发率、1 a生存率比较差异均无统计学意义(P>0.05);根据QLQ-BR23生存质量评分结果得知,保乳组的生存质量较改良组显著提高(P<0.05).结论保乳术可明显提高早期乳腺癌患者生存质量,安全性高,疗效满意,且符合人体美学的要求.Objective To analyze the effect of breast conserving surgery and modified radical mastectomy on theefficacy and life quality of early breast cancer.Method 60 cases of early breast cancer patients were divided intoimproved group and conservative group,intraoperative and short-term curative effect,aesthetic effect,the incidenceof postoperative complications,the quality of life between the two groups were compared. Results The operationtime,the amount of bleeding,the hospitalization time in the conservative group were significantly lower than those ofthe improved group （P〈0. 05）;the excellent rates of cosmetic effect in breast conserving group and improved groupwere 93. 3% （28 / 30）,60% （18 / 30）,the cosmetic effect of breast conserving group is superior （P〈0. 05）;theincidence of postoperative complications between breast conserving group and improved group were 13. 2% and43%,the difference was statistically significant （P〈0. 05）;1 year distant metastasis rate,local recurrence rate,1year survival rate of two groups after operation showed no significant difference （P〉0. 05）;according to the survivalquality of QLQ-BR23 score, results showed that the quality of life of breast conserving group compared withimproved group increased significantly （P〈0. 05）. Conclusion Breast conserving surgery can obviously improvethe life quality of the patients with early breast cancer,with high safety and satisfactory curative effect,which is inline with the requirements of human aesthetics.
