联合深层巩膜条切除的小梁切除术治疗开角型青光眼的效果分析

摘　　要：目的探讨联合深层巩膜条切除的小梁切除术治疗开角型青光眼的效果分析。方法回顾分析2010年2月—2017年2月收治61例61眼开角型青光眼患者,其中32眼予联合深层巩膜条切除的小梁切除技术为观察组,29眼采取了传统的小梁切除术为对照组,观察两组术后矫正视力﹑并发症以及眼压控制情况,比较两种手术方法的疗效情况。结果术后1年,观察组32眼中28眼(87.5%)功能性滤过泡存在,对照组19眼(65.5%)存在;术后2周及术后1年,观察组的平均眼压数值明显低于对照组,两组比较有显著性差异(P<0.05)。术中及术后两组均未发现严重的不良并发症,术后两组的矫正视力比较也无明显差异。结论较传统的小梁切除术治疗开角型青光眼,联合深层巩膜条切除的小梁切除术其长期的眼压控制更安全有效。Objective To evaluate the effects of trabeculectomy combined with subscleral sclerectomy in the treatment open angle glaucoma.Methods 61 patients with open angle glaucoma were treated during February 2010 and February 2017. 32 patients were treated with trabeculectomy combined with subscleral sclerectomy. 29 patients were treated with traditional trabeculectomy. We observed the difference in postoperative complications,postoperative visual and postoperative intraocular pressure of two groups. The efficacy of the two groups was compared.Results One year after the operation,postoperative bleb formations in 28 eyes（ 87. 5 %） of the observation group and 19 eyes（ 65.5 %） of the control group. Two weeks and one year after the operation,the average intraocular pressure of the observation group was significantly lower than that of the control group. The difference between the two groups was statistically significant（ P 0. 05）. No serious intraoperative and postoperative adverse complications were found in both groups, and there was no significant difference in correction vision between the two groups.Conclusions In the treatment of open angle glaucoma,trabeculectomy combined with subscleral sclerectomy could control intraocular pressure more safe and effective compared with traditional trabeculectomy.
