结核性胸膜瘤形成的危险因素分析

摘　　要：目的 探讨结核性胸膜炎患者胸膜瘤形成的危险因素.方法 选取2015年2月至2017年7月于晋城市第三人民医院确诊的结核性胸膜炎患者121例,其中出现胸膜瘤者36例.收集所有患者的一般资料、胸水常规、生化检查、胸部CT、胸水彩色多普勒超声检查,采用t检验、χ2检验及Logistic回归分析,分析胸膜瘤形成的危险因素.结果 单因素分析结果显示,男性发生结核性胸膜瘤的危险性高于女性,胸水乳酸脱氢酶（LDH）含量高、胸部CT提示胸膜肥厚、彩色多普勒超声提示胸水包裹、分隔是结核性胸膜瘤发生的危险因素（P〈0.05）.将危险因素纳入Logistic逐步回归分析法进一步行多因素分析,结果显示,胸水LDH值,胸部CT提示胸膜肥厚,彩色多普勒超声提示胸水分隔、包裹为结核瘤形成的独立危险因素.结论 胸水LDH含量高,彩色多普勒超声提示胸水分隔、包裹,胸部CT提示胸膜肥厚为结核瘤形成的危险因素.Objective To investigate the risk factors of pleural tumor development in patients with tuberculous pleurisy .Methods A total of 121 patients with tuberculous pleurisy diagnosed in the Third People＇s Hospital of Jincheng from February 2015 to July 2017 were selected as research objects , including 36 patients with pleural tuberculoma .The general data of all patients were collected , including pleural fluid routine examination , biochemical examination , chest CT and chest color Doppler ultrasonography .Logistic regression analysis , t test and card square test were used to analyze risk factors of pleural tumor .Results Univariate analysis showed that males had a higher risk of pleural tuberculoma than females , high content of lactate dehydrogenase （ LDH ） in pleural fluid , chest ultrasound showed pleural fluid separation and package , chest CT showed pleural thickening were risk factors of tuberculoma . Identified risk factors were put into Logistic regression for multivariate analysis , of which results showed LDH value in pleural fluid , pleural thickening by chest CT , pleural fluid separation and package by color Doppler ultrasound were independent risk factors of tuberculoma .Conclusions High content of LDH in pleural fluid suggests that pleural fluid separation and package and pleural thickening were risk factors of pleural tuberculoma according to color Doppler ultrasonography and chest CT .
