生大黄联合芒硝对重症胰腺炎患者的治疗效果

摘　　要：目的观察重症急性胰腺炎（SAP）患者在常规西医治疗的基础上加用生大黄灌肠联合芒硝腹部外敷的中药治疗的治疗效果。方法选择2013年1月至2016年12月常熟市第一人民医院重症医学科收治的SAP患者45例，将其分为生大黄联合芒硝加常规西医治疗组（23例）和常规西医治疗组（22例），监测患者治疗前和治疗后第5天血管外肺水指数（EVLWI）等脉搏指示连续心排量监测（PICCO）血流动力学指标，以及腹内压（IAP）、氧合指数（PO2／FiO2）。结果生大黄联合芒硝加常规西医治疗显著降低了SAP患者的EVLWI、每搏输出量变异度（SVV）、IAP（P〈0．05）；但两组心脏指数（CI）和氧合指数的改善情况比较差异未见统计学意义（P〉0．05）。结诊哇大昔联．会芒硝加常期．西医治疗☆}．改善SAP蘸者的吾B佘II盘廉指标和平面后．Objective To observe the therapeutic effect of traditional Chinese medicine on the treatment of severe acute pancreatitis （SAP） with raw rhubarb enema combined with mirabilite abdomen external application. Methods From January 2013 to December 2016, 45 cases of SAP in Intensive Care Unit of the People＇s Hospital of Changshu were collected and they were divided into two groups, one group was treated with conventional western medicine treatment and raw rhubarb, mirabilite （23 cases） , the other group was only treated with conventional western medicine treatment （22 cases）. Then the PICCO related hemodynamics index such as extravascular lung intra-abdominal pressure （ IAP）, oxygenation index （ PO2/FiO2 ） before monitored. Results Raw rhubarb combined with mirabilite and water index （ EVLWI ） and and 5 days after treatment were conventional western medicine significantly reduced the EVLWI, SVV, IAP of SAP patients （P 〈 0. 05） , but there was no significant difference in the improvement of the two groups of cardiac index （CI） and oxygen index （P 〉 0. 05 ）. Conclusions Raw rhubarb eombined with mirabilite and conventional Western medicine can improve some clinieal indexes and prognosis of SAP patients.
