摘  要:目的 探讨彩色多普勒超声对自发性孤立性肠系膜上动脉夹层(SISMAD)的诊断价值.方法 对经CT血管成像证实的19例SISMAD的声像图表现进行回顾性总结分析.结果 SISMAD为肠系膜上动脉增宽,可见强回声隔膜将管腔分为真假两腔,部分假腔内可见血栓形成,部分患者真腔受压,管腔明显变小,血流速度超过275 cm/s.19例夹层中Ⅰ型夹层5例,Ⅱa型为4例,Ⅱb型9例,Ⅲ型1例,其中18例患者彩色多普勒超声均做出明确诊断及正确分型.结论 彩色多普勒超声是诊断及随访SISMAD有价值的影像学检查方法.Objective To evaluate the clinical value of color Doppler ultrasonography in diagnosing spontaneous isolated superior mesenteric artery dissection (SISMAD). Methods The ultrasonographic images of 19 patients with SISMAD confirmed by CT angiography were retrospective analyzed and the ultrasonographic features were summarized. Results The ultrasonographic images of SISMAD showed that the lumens of superior mesenteric arteries were wider, the intimal flaps were found in superior mesenteric artery, the false lumens were filled with thrombus in some patients, the true lumens were compressed and significantly smaller, and the blood flow velocity would exceed 275 cm/s. Nineteen cases of SISMAD were divided into three types: type Ⅰ 5 cases, type Ⅱa 4 cases, type Ⅱb 9 cases and type Ⅲ 1 case. Eighteen of them were clearly diagnosed and classified by color Doppler ultrasonography. Conclusions Color Doppler ultrasonography could be an effective imaging method for the diagnosis and follow-up of SISMAD.
