活髓切断术联合透明成形冠即刻修复上颌乳前牙外伤的临床评价

摘　　要：目的:应用活髓切断术联合透明成形冠治疗上颌乳前牙外伤,随访对其临床效果进行评价。方法:选取外伤冠折露髓的上颌乳前牙62例,随机分为治疗组和对照组各31例,两组均行活髓切断术治疗,其中治疗组应用透明成形冠直接进行冠修复,对照组应用复合树脂进行冠修复,18个月内定期复查,观察并评价两组患牙的临床效果。结果:随访期内未见继发龋、冠折裂、根折。仅1颗出现根尖周炎症状,1颗自然脱落,树脂冠1颗色泽微变、3颗表面着色。结论:对于上颌乳前牙外伤冠折露髓应用活髓切断术联合透明成形冠的治疗方法,可最大限度保存患牙,外形美观,疗效显著。值得临床推广应用。Objective： To evaluate the clinical effect of the treatment of maxillofacial anterior teeth trauma with pulp dissection and transparent crown. Methods： A total of 62 cases of maxillary lacteal anterior teeth with broken crown and exposed medulla were randomly divided into cases in treatment group and control group with 31 cases in each. Both groups were treated with live myelotomy, in which the treatment group was treated with transparent shaped crown directly, and the control group was treated with composite resin. Periodic reexamination within 18 months was conducted to observe and evaluate the clinical effects of the two groups. Results： No secondary caries, crown fractures, or root fractures were observed during follow-up. Only one of them showed periapical inflammation, and one of them fell off naturally. The color of one of the resin crowns changed slightly and the surface of three of them was stained. Conclusion： For the treatment of the injured maxillary anterior teeth with broken crown and exposed pulp, the treatment method of combined pulp dissection and transparent crown can save the affected teeth to the maximum extent, with beautiful appearance and remarkable curative effect, which is worthy of clinical promotion and application.
