子宫内膜癌组织中hTERC基因表达及临床意义

摘  要:目的:探讨h TERC基因在子宫内膜癌组织表达情况,探讨该基因在子宫内膜癌作用的价值。方法:收集正常子宫内膜组织和子宫内膜癌手术标本各25例,采用FISH技术检测石蜡包埋切片组中h TERC基因扩增情况。结果:正常子宫内膜、子宫内膜样腺癌h TERC基因阳性表达率分别为20%(5/25)、36%(9/25),子宫内膜癌组织阳性率高于正常子宫内膜组,但经χ~2检验两组h TERC基因扩增阳性率比较差异无统计学意义(χ~2=1. 587 3,P=0. 207 7)。按子宫内膜癌临床分期,Ⅰ/Ⅱ期、Ⅲ/Ⅳ期h TERC基因阳性率为40%(8/20)、20%(1/4),临床期别早h TERC基因扩增比晚期病例增高,经统计学检验Fisher计算后,差异无统计学意义(P=0. 620 6)。按子宫内膜样腺癌分化程度,高分化、中分化、低分化h TERC基因阳性表达率分别为22. 22%(2/9)、50%(4/8)、37. 5%(3/8),随着腺癌分化程度越差,h TERC基因扩增阳性率增高,但经过Fisher确切概率法计算后差异无统计学意义(P=0. 516 5)。结论:子宫内膜癌患者h TERC基因有扩增趋势,癌细胞分化程度越差h TERC基因阳性率增加,预测子宫内膜病变的高风险。Objective The aim of this study was to investigate the expression of Human telomerase RNA component (hTERC) gene in endometrial carcinoma and its clinical application.〖HTH〗 Method 25 cases of normal endometrial tissue and 25 cases of endometrial carcinoma were collected.The hTERC gene amplification in paraffin embedded sections were detected by Fluorescence in situ hybridizatio(FISH)technique.Results The positive rate of hTERC gene in normal en dometrium and endometrial adenocarcinoma were respectively 20% (5/25),36% (9/25),respectively,the positive rate inendometrial carcinoma was higher than innormal endometrium group,but the two groups by chi square test hTERC gene amplification positive rate difference was not statistically significant ( χ 2 =1.587 3, P=0.207 7).According to the clinical staging of endometrial cancer,the positiverate of hTERC gene in Ⅰ/Ⅱ stage and Ⅲ/Ⅳ stage were respectively 40% (8/20),20% (1/4),the clinical early stage hTERC gene amplification positive was higherthan that of advanced cases,but after Fisher exact probability calculation,and there was no significant difference ( P =0.620 6).In endometrioid adenocarcinoma differentiation,high differentiation,differentiation and low differentiation of hTERC gene positive expression rate was 22.22% (2/9),50% (4/8),37.5% (3/8),with the degree of differentiation of adenocarcinoma was poor,the positive rate of hTERC gene amplification increased,but after Fisher exact probability calculation method no statistical significance ( P =0.516 5). 〖WTHZ〗Conclusion Endometrial cancer patients with hTERC gene amplification trend,the degree of differentiation of cancer cells hTERCgene positive rate increased,it predict the high risk of endometrial lesions.
