保留左结肠动脉应用于腹腔镜低位直肠前切术的临床研究

摘　　要：目的:研究保留左结肠动脉在腹腔镜低位直肠前切术中的应用价值。方法:选择接诊的直肠癌病患58例,将之利用奇偶数字分组法随机分成试验和对照两组,每组29例。两组都进行腹腔镜低位直肠前切术治疗,试验组在术中保留左结肠动脉,对照组不保留左结肠动脉。综合分析两组手术治疗的效果,比较肠系膜淋巴结清扫数量等指标。结果:试验组的手术用时、清扫淋巴结数量以及术中出血量与对照组比较,差异无统计学意义(P> 0. 05)。试验组的术后吻合口漏发生率为0,明显比对照组的10. 34%低,组间差异显著有统计学意义(P <0. 05)。试验组的回肠造口率和对照组比较更低,组间差异显著有统计学意义(P <0. 05)。结论:于腹腔镜低位直肠前切术中积极应用保留左结肠动脉方案,可确保患者近端肠管的血运,降低吻合口漏发生率,提高预后效果。Objective To study the application value of left colon artery in laparoscopic low rectal excision. Method 58 cases of rectal cancer patients were selected in this study,they were randomly divided into two groups using ode1 even numbers ：29 patients in each group. Both groups were treated with laparoscopic low rectal excision. The experimental group retained the left co- lon artery in the operation, and the control group did not retain the left colon arlery. The effect of lwo groups of surgical treatment was analyzed, and file number of mesenteric lymph node dissection was compared. Results There was no significant difference belween the operation time of the experimental group, the number of clearing lymph nodes, and the amount of intraoperative blood loss and the control group （P 〉 0. 05）. The incidence of postoperative anastomotic leakage in the experimental group was 0. 0% , which was significantly lower than 10. 34% in the control group, and the difference was statistically significant （P 〈 0. 05 ）. In the experimental group,the rate of ileostomy was lower compared with that in the control group,and the difference between groups was statistically significant （ P 〈 0. 05 ）. Conclusion The left colonic artery can he used to ensure the blood flow of the proximal intestinal canal and reduce the incidence of anastomotic leakage and improve the prognosis.
