湿疹患者大鱼际掌纹特应征的临床研究

摘　　要：目的观察湿疹患者的大鱼际掌纹特应征,为湿疹的防治提供临床依据。方法以171例湿疹患者为观察组,并按1∶1匹配性别、年龄相同的健康体检者作为对照组,观察所有研究对象大鱼际掌纹特征并进行分级,统计观察组患者病程并进行过敏原检测,分析大鱼际掌纹特征与年龄、病程、过敏原的关系。结果不同大鱼际掌纹形态分级者的年龄、病程比较差异均无统计学意义(P均> 0. 05); Logistic回归分析示,大鱼际掌纹特征是湿疹患者的危险因素(OR=29. 750,95%CI[10. 985,80. 572]);湿疹患者大鱼际掌纹形态分级与谷类、牛奶、花粉阳性程度正相关(rp=0. 591,0. 692,0. 593,P均<0. 05)。结论大鱼际掌纹特应征是湿疹的典型体征,该体征不随病程的长短而变化,相对恒定,大鱼际掌纹特应征呈阳性的湿疹患者应尽量避免接触谷类、牛奶、花粉。Objective It is to observe the special indications of large thenar eminence palm print in eczema patients, and provide clinical evidence for the prevention and treatment of eczema.Methods One hundred and seventy-one patients with eczema were selected as observation group, and the healthy persons with the same sex and age were selected as the control group according to 1：1. The characteristics of large thenar eminence palm print in all subjects were observed and classified, and the course of disease was counted and the allergen were detected, the relationship between large thenar eminence palm print characteristics and age, course of disease and allergens were analyzed.Results There was no significant difference in age and course of disease among the patients with different morphological grading of thenar palm print （ P 〉0.05）. Logistic regression analysis showed that the special indications of large thenar eminence palm print was a risk factor for eczema （OR=29.750, 95% CI [10.985,80.572]）. The morphological grading of large thenar eminence palm print in eczema patients was positively correlated with the positive degree of cereals, milk and pollen （ r p=0.591, 0.692, 0.593, P 〈0.05）.Conclusion The special indications of large thenar eminence palm print is a typical sign of eczema. This sign does not vary with the duration of the disease, but is relatively constant. Patients of eczema with positive special indications of large thenar eminence palm print should avoid contact with cereals, milk and pollen as far as possible.
