前伸下颌法确定牙合平面在全口义齿修复中的应用

摘　　要：目的:探讨前伸下颌法确定全口义齿牙合平面的临床效果。方法:选择28～78岁的无牙颌患者100例,用嵴唇距确定牙合平面前点的方法制作全口义齿。在全口义齿修复后1、3个月时,从固位情况、咀嚼功能、外观、发音的影响、舒适度等5个方面评价全口义齿的修复效果。结果:在100例无牙颌患者中,1 min确定牙合平面前点为56例,2 min为34例,3 min为10例;嵴唇距类型为极短距14%,短距20%,中等距41%,长距15%,极长距10%。全口义齿修复后1个月,患者对全口义齿的外观、发音的影响、咀嚼功能、固位情况、舒适度的满意度分别为88. 0%、85. 0%、59. 0%、68. 0%和70. 0%,修复后3个月分别为89. 0%、89. 0%、63. 0%、76. 0%和73. 0%,1、3个月相比除外观无显著差异(P> 0. 05)外,其余均有显著差异(P <0. 05)。结论:采用前伸下颌法定位牙合平面前点进行全口义齿的制作,患者满意度良好。AIM： To evaluate the application of the determination of the anterior point of maxillary full denture occlusal plane by the protrusion of mandible to the upper lip edge. METHODS： 100 cases of edentulous patients with denture occlusal reconstruction were included. The distance between mandile anterior alveolar crest to the lower edge of the upper lip was used to determine the anterior point of maxillary full denture occlusal plane, the dentures were conventionally manufactured. Followed up was conducted for 1-3 months. RESULTS： The front of occlusal plane was identified in 1 min for 56 patients, in 2 min for 34 and in 3 min for 10. And the very short, short, medium, long and very long distance accounted for 14%, 20%, 41%, 15% and 10% respectively. At the first month follow-up, the satisfaction rate of appearance, language, chewing, retention and comfort were 88.0%, 85.0%, 59.0%, 68.0% and 70.0%, at the third months follow-up were 89.0%, 89.0%, 63.0%, 76.0% and 73.0%, respectively （ P 〈0.05 ）. CONCLUSION： The new method of making the complete denture with mandible protrusion to determine the front point of the occlusal plane can improve the repair quality of the complete denture.
