口炎清颗粒联合曲安奈德对口腔扁平苔藓患者血清IL-2、TNF-α水平的影响

摘　　要：目的:探讨口炎清颗粒联合曲安奈德(TA)治疗口腔扁平苔藓(OLP)的临床效果及对患者患者血清白细胞介素(IL)-2、肿瘤坏死因子(TNF)-α水平的影响。方法:选取我院2015年3月～2017年3月收治的110例OLP患者,依据随机数字表法均分为两组。对照组采取TA局部注射治疗,观察组在对照组基础上联合口炎清颗粒治疗。治疗4周后,比较两组的临床疗效,治疗前后血清IL-2、TNF-α水平、口腔健康影响程度量表(OHIP)-14评分的变化及不良反应的发生情况。结果:治疗4周后,观察组总有效率达92.7%,较对照组明显上升(78.2%,P<0.05)。两组治疗4周后血清IL-2、TNF-α水平、OHIP-14评分较治疗前均显著降低(P<0.01),且观察组血清IL-2、TNF-α水平、OHIP-14评分均显著低于对照组(P<0.01)。两组不良反应发生率对比差异无统计学意义(P>0.05)。结论:口炎清颗粒联合曲安奈德可显著提高口腔扁平苔藓的临床疗效,提高患者生活质量,且安全性高,可能与其降低患者血清IL-2、TNF-α水平,抑制炎症反应有关。Objective：To investigate the effect of kouqingyan granule combined with triamcinolone acetonide（TA） on the oral lichen planus（OLP） and serum IL-2 and TNF-α levels.Methods：110 cases of patients with OLP who admitted in our hospital from March 2015 to March 2017 were selected and randomly divided into two groups.The control group was treated with local injection with TA,and the observation group was treated with kouqingyan granule on the basis of control group.Then the clinical effect,serological indicators,oral health impact profile（OHIP） and the incidence of adverse reactions of the two groups were observed and compared before and after the treatment.Results：At 4 weeks after treatment,the overall effective rate of observation group was 92.7%,which was higher than 78.2% in the control group（P〈0.05）.The serum IL-2 and TNF-α levels of both groups were decreased than those before treatment（P〈0.01）,which were significantly higher in the observation group than those of the control group（P〈0.01）.The OHIP-14 scores of both groups at 4 weeks after treatment declined significantly than those before treatment（P〈0.01）,which was evidently lower in the observation group than that of the control group（P〈0.01）.No difference was found in the incidence of adverse reactions between the two group（P〉0.05）.Conclusion：The combination of kouqingyan granule and triamcinolone acetonide can relieve the symptoms,control the inflammatory response and improve the quality of life more quickly in the treatment of oral lichen planus.
