摘  要:目的:探讨三维容积超声断层成像技术(TUI)在诊断腮腺占位性病变中的应用价值。方法:对52例病人的55个腮腺占位性病变均行常规二维超声检查,然后应用三维超声TUI技术检查,将两种检查方法的诊断结果分别与病理结果进行对比分析,比较两种方法在腮腺占位性病变诊断中的敏感度、准确率、特异度、阳性预测值、阴性预测值。结果:二维超声诊断与病理诊断符合率为76.3%(42/55),三维超声TUI技术诊断与病理诊断符合率为91.0%(50/55,P <0.05);三维超声TUI技术诊断腮腺恶性占位性病变的敏感度为94.7%(36/38)、特异度为82.4%(14/17)、准确率为91.0%(50/55)、阳性预测值为92.3%(36/39)、阴性预测值为87.5%(14/16),两种检查方法对腮腺占位性疾病诊断差异有统计学意义(P<0.05)。结论:三维超声TUI技术可以更直观立体地观察腮腺病灶的形态、位置、浸润深度及与周边结构的关系,对鉴别病灶的良恶性提供更加丰富的诊断信息,具有较高的临床应用价值。Objective: To investigate the value of three-dimensional volume tomography ultrasound imaging (TUI) in the diagnosis of parotid space occupying lesions. Methods: 55 cases of parotid space occupying lesions are examined in 52 patients by conventional two-dimensional ultrasound and then examined by three-dimensional TUI technique, the diagnostic results of the two methods are compared with the pathological results, respectively, the sensitivity, accuracy, specificity, positive predictive value and negative predictive value are compared between the two methods in diagnosis ofparotid space occupying lesions. Results: The coincidence rate with pathology for 2D-ultrasound is 76.3% (42/55), for three-dimensional ultrasound TUI technique is 91.0% (50/55, P〈0.05). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of three-dimensional ultrasound TUI technique are 94.7% (36/38), 82.4% (14/17), 91.0% (50/55), 92.3% (36/39), 87.5% (14/16) in diagnosis of malignant lesions ofparotid gland, the difference between the two methods in the diagnosis of parotid space occupying lesions is statistically siga^ificant (P〈0.()5). Conclusion: three-dimensional ultrasound TUI technique can be used to observe the shape, location, depth of invasion and the relationship with the surrounding structures more intuitive and three-dimensional, it can provide more abundant diagnostic information for diffrerentiating benign and malignant lesions, which has high clinical application value.
