泪道内窥镜下双管置入术治疗慢性泪囊炎的疗效及影响因素分析

摘　　要：目的探讨应用泪道内窥镜联合双管置入术治疗慢性泪囊炎的临床疗效及其相关影响因素。方法回顾性选择2013年1月～2014年5月于沈阳市第四人民医院眼科就诊的并行泪道内窥镜下双管置入术的139例（152只眼）慢性泪囊炎患者。记录性别、年龄、病程及术后拔管时间、术后疗效，统计分析其相关影响因素。数据采用秩和检验或Spearman相关检验。结果术后随访6～12个月，139例（152只眼）慢性泪囊炎患者中治愈130只眼（85．5％），好转8只眼（5．3％），无效14只眼（9．2％），总有效率90．8％。所有患者均无明显手术并发症发生。单因素分析发现，不同性别及不同病程的患者其手术治疗效果差异无统计学意义（P，0．05），而不同年龄、不同拔管顺序、不同术后拔管时间的患者其手术治疗效果差异均有统计学意义。结论应用泪道内窥镜联合双管置入术治疗慢性泪囊炎，成功率较高，且无明显手术并发症。术前充分考虑患者不同年龄对手术效果的影响，术后定期随诊并严格控制拔管顺序及拔管时间，可以进一步提高手术成功率。Objective To explore the clinical curative effects and its related factors of lacrimal duct intubation with double tube under lacrimal endoscope for treating chronic dacryocystitis. Methods From January 2013 to May 2014, 139 patients （152 eyes） with chronic dacryocystitis received lacrimal duct in-tubation with double tube under lacrimal endoscope were collected from department of ophthalmology of the fourth people＇s hospital of Shenyang. Gender, age, course, extubation time and curative effect were recorded. Statistics were used to analyze the relevant factors. The fmal data using Mann-Whitney rank test or Spearman correlation test. Results After postoperative follow-up 6-12 months, 139 cases （152 eyes） in patients with chronic dacryocystitis cured 130 eyes （85.5%）, improvement in 8 eyes （5.3%）, in- valid 14 eyes （9.2%）, the total effective rate was 90.8%. All patients had no obvious complications. Sin-gle factor analysis showed that the curative effects in patients with different gender and different course had no statistical significance （P 〉0.05）, while in patients with different ages, different extubation se-quence and different extubation time the difference all had statistical significance. Contusions The cu-rative rate of lacrimal duct intubation with double tube under lacrimal endoscope for treating chronic dac-ryocystitis is high without obvious complication. Preoperative fully consider the impact on curative ef-fects in patients of different ages, and after extubation regularly followed-up and strictly control the extu-bation order and extubation time, it can further improve the success rate.
