针刺治疗睑板腺功能障碍相关干眼的疗效及对泪液中CXCR3和CXCL10含量的影响

摘　　要：目的观察针刺治疗睑板腺功能障碍(meibomian gland dysfunction,MGD)所致干眼的疗效及对泪液中CXC趋化因子受体3(CXC chemokine receptor 3,CXCR3)和CXC趋化因子配体10(CXC chemokine ligand 10,CXCL10)含量的影响。方法选择2017年1月至2018年5月在北京中医药大学第三附属医院眼科门诊确诊为MGD所致干眼患者60例(120眼),随机分为2组,每组30例(30只眼,选症状较重的一只眼),分别给予人工泪液及双氯芬酸钠滴眼液治疗(对照组)或针刺治疗(治疗组)。治疗前及连续治疗3周后,分析比较两组患者泪膜破裂时间(break up time,BUT),泪液分泌试验(Schirmer I test,SIT)以及泪液中CXCR3和CXCL10的含量。结果治疗组治疗总有效率(86.67%)高于对照组(70.00%),差异有显著性(P<0.05)。两组患者治疗前BUT及SIT比较差异无显著性(P>0.05);治疗后,两组患者BUT及SIT均高于治疗前,且治疗组BUT高于对照组,差异有显著性(P<0.05)。两组患者治疗前泪液中CXCR3、CXCL10含量比较差异无显著性(P>0.05);治疗后,两组患者泪液中CXCR3、CXCL10含量均低于治疗前,且治疗组均低于对照组,差异有显著性(P<0.05)。结论针刺治疗MGD相关干眼疗效显著,对患者的BUT、SIT以及泪液中CXCR3、CXCL10含量均有明显改善作用。Objective To evaluate the effects of acupuncture treatment on the CXC chemokine receptor 3（CXCR3） and CXC chemokine ligand 10（CXCL10） of dry eyes caused by meibomian gland dysfunction（MGD）. Method Sixty patients suffered from dry eyes were collected and divided into 2 groups randomly by computer, which were treated either by artificial tears and non-steroidal anti-inflammatory eye drops therapy（control group）or acupuncture treatment（treatment group）respectively. The tear break-up time（BUT）, Schirmer I test（SIT）, the content of CXCR3 and CXCL10 in the tears of patients were detected and marked before treatment and 3 weeks after treatment. Result After 3 weeks, the SIT, BUT, the content of CXCR3 and CXCL10 in the tears in the treatment group and control group are better than that before treatment（P〈0.05）.Compared with the control group the BUT, the content of CXCR3 and CXCL10 in the tears of the treatment group were better（P〈0.05）.But there is no significant difference in SIT between the two groups（P〉0.05）. Conclusion The study found that acupuncture treatment could improve the level of BUT, SIT and reduce the content of CXCR3 and CXCL10 in the tears of dry eye patients caused by MGD. The curative effect of acupuncture is better than the control group.
