半夏厚朴清心汤联合经皮穴位电刺激治疗癔球症的临床疗效

摘　　要：目的探讨半夏厚朴清心汤联合经皮穴位电刺激(TEAS)治疗癔球症的临床疗效。方法选取广州市第一人民医院南沙医院消化专科门诊癔球症患者75例,经知情同意后随机分为常规对照组(泮托拉唑,40 mg/d)、半夏厚朴汤组、半夏厚朴清心汤组、联合治疗组(半夏厚朴清心汤+TEAS),各组15例,疗程均为4周。在治疗前后及过程中,采用高分辨率食管测压(HRM)进行食管上括约肌(UES)及食管下括约肌(LES)检测,结合癔球症症状评分(GETS)、汉密尔顿抑郁/焦虑量表(HAMD/HAMA)及匹兹堡睡眠质量指数(PSQI)进行疗效评估。结果治疗4周后,联合治疗组UES、GETS评分、HAMD/HAMA评分及PSQI评分均较治疗前明显改善,且优于其余各组,差异有统计学意义(P <0. 05)。结论联用半夏厚朴清心汤及TEAS治疗癔球症具有良好的临床疗效,为进一步临床推广提供有力证据。Objective To investigate the clinical therapeutic effect of the combination of modified Banxia Houpu decoction and transcutaneous electrical acupoint stimulation（ TEAS） on globus pharyngeus. Methods After informed consent,75 out-patients with globus pharyngeus from Department of Gastroenterology in Guangzhou First People＇s Hospital Nansha Hospital were randomly divided into normal control group（ pantoprazole,40 mg/d）,Banxia Houpu decoction group,modified Banxia Houpu decoction group,TEAS group,and combinative treatment group（ modified Banxia Houpu decoction ＋ TEAS）,with 15 cases in each group and a 4-weeks treatment course. Before and after the treatment,high-resolution manometry（ HRM） was used to measure the pressure of upper oesophageal sphincter（ UES） and lower esophageal sphincter（ LES）,meanwhile Glasgow-Edinburgh Throat Scale（ GETS）,Hamilton Rating Scale Anxiety/Depression（ HAMD/HAMA） and Pittsburgh Sleep Quality Index（ PSQI） were used to assess the therapeutic effects.Results After 4 weeks of treatment,UES,GETS,HAMD/HAMA and PSQI were obviously improved in combinative treatment group,and superior to other groups,with significant difference（ all P〈0. 05）. Conclusion The combination of modified Banxia Houpu decoction and TEAS exhibited a good clinical therapeutic effect on globus pharyngeus,and it provides strong evidence for further clinical promotion.
