摘 要:目的探讨急性前循环梗死患者血清中单核细胞趋化蛋白-1(MCP-1)和血管内皮细胞钙黏蛋白(VEcadherin)水平变化,并分析其临床意义。方法选择2016年4月至2017年1月安阳市第三人民医院收治的145例急性前循环脑梗死患者为研究对象(脑梗死组),另选择同期体检健康者50例作为对照组。采集2组受试者空腹肘静脉血,采用酶联免疫吸附试验检测血清中MCP-1及VE-cadherin水平,采用颗粒免疫透射比浊法检测脑梗死组患者血清中同型半胱氨酸(Hcy)、胱抑素(Cys C)及尿酸(UA)水平。脑梗死组患者根据病情分为进展性脑梗死组(n=43)和非进展性脑梗死组(n=102),根据患者梗死病灶分为完全性前循环梗死组(n=35)和非完全性前循环梗死组(n=110),根据MCP-1和VE-cadherin水平将脑梗死组患者又分为高MCP-1组(MCP-1≥315. 9 ng·L-1,n=72)、低MCP-1组(MCP-1 <315. 9 ng·L-1,n=73)和高VE-cadherin组(VE-cadherin≥6. 0 mg·L-1,n=72)、低VE-cadherin组(VEcadherin <6. 0 mg·L-1,n=73)。比较脑梗死组和对照组受试者血清MCP-1、VE-cadherin水平及各亚组患者血清MCP-1、VE-cadherin或Hcy、Cys C及UA水平。结果脑梗死组患者血清MCP-1和VE-cadherin水平均高于对照组(P <0. 05)。进展性脑梗死组患者血清MCP-1和VE-cadherin水平均高于非进展性脑梗死组(P <0. 05);完全性前循环梗死组患者血清MCP-1和VE-cadherin水平均高于非完全性前循环梗死组(P <0. 05);高MCP-1组患者血清Hcy、Cys C及UA水平均显著高于低MCP-1组(P <0. 05);高VE-cadherin组患者血清Hcy、Cys C及UA水平均显著高于低VE-cadherin组(P <0. 05)。结论急性前循环脑梗死患者血清中MCP-1和VE-cadherin水平高于健康人群,且MCP-1和VE-cadherin水平可反映脑梗死患者的病情及临床类型。Objective To investigate the clinical significance of the changes of the levels of serum monocyte chemokine-1 (MCP-1) and vascular endothelial-cadherin (VE-cadherin) in acute anterior circulation cerebral infarction patients.Methods One hundred and forty-five patients with acute anterior circulation cerebral infarction in the Third People′s Hospital of Anyang City from April 2016 to January 2017 were selected as the study objects (cerebral infarction group);another 50 healthy person in the same period were selected as control group.The fasting elbow venous blood of all subjects was collected.The levels of serum MCP-1 and VE-cadherin of all subjects were detected by enzyme-linked immunosorbent assay;the serum levels of homocysteine (Hcy),cystatin C (CysC) and uric acid (UA) of patients in the cerebral infarction group were detected by particle immunoturbidimetry.The patients in the cerebral infarction group were divided into progressive cerebral infarction group ( n =43) and non progressive cerebral infarction group ( n =102)according to patient′s condition,they were divided into complete anterior circulation cerebral infarction group ( n =35) and incomplete anterior circulation cerebral infarction group ( n =110)according to patient′s infarction lesions,they were divided into high MCP-1 group (MCP-1≥315.9 ng·L -1 , n =72),low MCP-1 group (MCP-1〈315.9 ng·L -1 , n =73) and high VE-cadherin group (VE-cadherin≥6.0 mg·L -1 , n = 72),low VE-cadherin group (VE-cadherin〈6.0 mg·L -1 , n =73) according to the median level of MCP-1 and VE-cadherin.The serum levels of MCP-1,VE-cadherin were compared between the cerebral infarction group and control group;and the levels of MCP-1,VE-cadherin or Hcy,CysC and UA were compared among the subgroups.Results The serum levels of MCP-1 and VE-cadherin of patients in cerebral infarction group were significantly higher than those in the control group ( P 〈0.05).The serum levels of MCP-1 and VE-cadherin of
