微血管减压术治疗原发性三叉神经痛手术技巧改进及临床疗效分析

摘　　要：目的探讨微血管减压术(MVD)治疗原发性三叉神经痛的手术技巧及临床疗效。方法回顾性分析2008年10月—2017年12月应用MVD治疗原发性三叉神经痛1 126例的临床资料,对其疗效进行分析。结果术后疼痛立即消失1 040例(92.4%);疼痛大部分缓解32例(2.8%);症状减轻36例(3.2%);无效18例(1.6%)。术后随访2个月～10年,88例患者7个月～6年复发(7.8%),予以射频治疗或再次微血管减压术后疼痛消失。结论 MVD治疗原发性三叉神经痛安全有效、创伤小、并发症少、复发率低,是目前治疗原发性三叉神经痛的首选方法。Objective To investigate the surgical technique and clinical efficacy of microvascular decompression（MVD） in the treatment of primary trigeminal neuralgia. Methods The clinical data of 1 126 cases of primary trigeminal neuralgia treated with MVD from October 2008 to December 2017 were retrospectively analyzed. Results Postoperative pain disappeared immediately in 1 040 cases（92.4%）; pain was mostly relieved in 32 cases（2.8%）; symptom reduction was 36 cases（3.2%）; ineffective 18 cases（1.6%）. After 2 to 10 years of follow-up, 88 patients relapsed（7.8%） from July to 6 years. The pain disappeared after radiofrequency treatment or microvascular decompression. Conclusion MVD is a safe and effective treatment for primary trigeminal neuralgia with less trauma, less complications and low recurrence rate. It is the first choice for the treatment of primary trigeminal neuralgia.
