摘 要:目的系统评价热清洗机机械清洗与手工清洗对牙科手机清洗效果和损耗的影响。方法计算机检索The Cochrane Library、The Joanna Briggs Institute Library、Embase、PubMed、MEDLINE、中国生物医学文献服务系统(CBM)、中国知网(CNKI)、维普(VIP)、万方数据库中关于手工清洗与机械清洗牙科手机效果的随机对照试验,检索时限均为建库至2018年2月。由2名研究员独立筛选文献、评价文献质量、提取资料,对符合质量标准的随机对照试验进行Meta分析。结果共纳入7篇文献,1140支牙科手机。Meta分析结果显示,机械清洗组清洗不合格率(OR=0.06,95%CI:0.01~0.72,P=0.03)和细菌培养阳性率(OR=0.04,95%CI:0.00~0.31,P=0.002)显著低于手工清洗组;在手机损耗方面差异无统计学意义(OR=0.30,95%CI:0.09~0.99,P=0.05)。结论热清洗机机械清洗能够改善牙科手机的清洗质量,优于传统的手工清洗;但在维护牙科手机的使用性能以及降低手机损耗方面还需要更多的研究予以证实。Objective To evaluate the effects of the mechanical cleaning of automatic hot cleaning machine and manual cleaning on dental handpieces via Meta-analysis. Methods A computerized literature search was carried out in the Cochrane Library,the Joanna Briggs Institute Library,Embase,PubMed,MEDLINE,CBM(Chinese Biomedical Database),CNKI(Chinese National Knowledge Infrastructure),VIP and Wanfang,to collect randomized controlled trails(RCTs)published from inception to February 2018 concerning the effects of the mechanical cleaning and manual cleaning in dental handpieces. Based on the inclusion and exclusion criteria,documents screening,literature quality assessment and data extraction were conducted by two researchers independently. Then Meta-analysis was performed for eligible RCTs. Results Totally 7 literatures were included involving 1140 handpieces. The results of Meta-analysis showed that the unqualified rate of cleaning(OR = 0.06,95%CI:0.01 ~ 0.72,P = 0.03)and the positive rate of bacterial culture(OR =0.04,95%CI:0.00 ~ 0.31,P = 0.002)in the mechanical cleaning group of automatic hot cleaning machine were significantly lower than those of the manual cleaning group. The difference had no statistical significance in the loss of dental handsets(OR = 0.30,95%CI:0.09 ~ 0.99,P = 0.05)between the two groups. Conclusion The mechanical cleaning of automatic hot cleaning machine can improve the cleaning quality of dental handpieces,which is superior to the traditional manual cleaning. However,more studies are needed to verify whether the mechanical cleaning is able to maintain the usability and reduce the loss of dental handsets.
