摘 要:目的探讨同型半胱氨酸(Hcy)和人乳房珠蛋白(hMAM)基因、myc基因和乳腺癌易感基因-1(BRCA-1)在乳腺癌中的表达和临床意义。方法选取2014年6月至2016年6月成都航天医院门诊和住院乳腺疾病患者165例,按疾病严重程度分为良性乳腺病组(58例)和乳腺癌组(107例),同时选取健康体检女性40例作为对照组,采用循环酶法检测3组Hcy水平,采用荧光定量聚合酶链反应(FQ-PCR)检测hMAM、BRCA-1和myc基因表达量。结果良性乳腺病组Hcy水平和h MAM、BRCA-1、myc基因表达与对照组比较,差异均无统计学意义(P>0.05);乳腺癌组BRCA-1基因表达显著低于对照组和良性乳腺病组,而Hcy水平和hMAM、myc基因表达显著高于对照组和良性乳腺疾组,差异均有统计学意义(P<0.05)。Hcy和h MAM、BRCA-1、myc基因联合检测乳腺癌的灵敏度和阴性预测值显著高于单个指标(P<0.05)。结论 Hcy对乳腺癌的辅助诊断有一定价值,若与hMAM、BRCA-1和myc基因联合检测,可有效提高对乳腺癌诊断效果。Objective To investigate the expression and clinical significance ofhomocysteine (Hcy) and human mammaglobin (hMAM) gene, myc gene and Breast cancer susceptibility gene-1 (BRCA-1) in breast cancer. Methods 165 outpatients and inpatients with breast disease in the Chengdu Aerospace Hospital from June 2014 to June 2016 were selected and divided into the benign breast disease group (58 cases) and breast cancer group (107 cases), meanwhile 40 females undergoing physical examination were selected as the control group. The Hey level was detected by adopting the circulating enzymatic method. The expression levels of hMAM, BRCA- 1 and mye gene were detected by fluorescence quantitative polymerase chain reaction. Results Compared with the control group, the Hey level, and expression levels of hMAM, BRCA-1 and mye gene in the benign breast disease group had no statistical difference (P〉0.05) ;the BRCA-1 gene expression level in the breast cancer group was significantly lower than that in the control group and benign breast disease group, while the Hey level, and expression levels of hMAM and myc gene in the breast cancer group were significantly higher than those in the control group and benign breast disease group, and the difference was statistically significant (P〈0.05).The sensitivity and negative predictive value in the combined detection of Hey, hMAM, BRCA- 1 and myc were significantly higher than those of single index. Conclusion Hey has a certain value for the adjuvant diagnosis of breast cancer.Its combined detection with hMAM, BRCA-1 and myc gene can effectively increase the diagnostic effect for breast cancer.
