白内障1063眼超声乳化术后低视力的影响因素

摘　　要：目的观察超声乳化联合人工晶状体植入术的临床效果及术后低视力的影响因素。方法采用连续序列临床病例回顾性研究。年龄相关性白内障840例（1063眼）接受超声乳化单焦点IOL植入术。患者平均年龄（67．8±10．5）岁，女性占56．9％。结果93．3％术后视力提高，5．1％视力无变化，1．6％视力下降。术前视力（10gMAR）为1．12±0．70，术后为0．30±0．35（t=41．660，P〈0．001）。80岁以上者143例，其中术后视力t〉0．3者占83．2％。糖尿病性视网膜病变、年龄相关性黄斑变性、青光眼、弱视、高龄和术后角膜水肿是术后低视力的影响因素（P〈0．05）。结论超声乳化单焦点IOL植入术安全有效，但存在上述影响因素。Objective To observe the clinical efficacy and the influence factors of low vision in patients with age-related cataract after phacoemulsification combined with IOL implantation. Methods Clinical data of 1 063 eyes of 840 cases with age-related cataract was analyzed in the retrospective clinical follow-up study. All cases received phaeoemulsification combined with IOL implantation. The mean age of patients was （67.8 ± 10.5 ） years, and 478 cases （ 56.9% ） were female. Results The visual acuity improved after surgery in 992 cases （93.3%） , unchanged in 54 cases （5.1%） , and decreased in 17 cases （ 1.6% ）. The visual acuity （logMAR） before surgery was 1.12 ±0.70, and the visual acuity after surgery was 0.30 ± 0.35 （t =41.66, P 〈 0.001 ）. Among them aged i〉80 years was in 143 patients, the postoperative visual acuity was ≥0.3 in 83.2%. The influence factors of low vision after surgery included diabetic retinopathy, age-related macular degeneration, glaucoma, amblyopia, postoperative corneal edema and advanced age. Conclusion Phacoemulsification combined with single focus IOL implantation is safe and effective, but there is some influence factors of low vision.
