泪小管断裂吻合术两种支撑管的效果比较

摘　　要：目的比较RS型泪道再通管与泪道引流管在泪小管断裂吻合术中应用的效果。方法泪小管断裂吻合术78例（78眼），随机分成两组，使用RS型泪道再通管作为泪管支撑材料者为A组，38例；使用泪道引流管者为B组，40例；两组患者均在术后3个月拔管。比较两组患者手术时间、术中舒适度、有效率和并发症发生率。结果A组手术时间（61．71±8．79）min，B组（68．25±7．56）min，差异有统计学意义（t=3．527，P=0．001）；术中疼痛评分：A组（3．58±0．83）分；B组（4．20±0．97）分（t=3．044，P=0．003）；两组并发症发生率A组15．8％；B组20．0％（χ^=0．235，P=0．656）。术后3个月拔管时评估：A组有效率92．1％，B组有效率90．0％（χ^2=0．106，P=0．752）。结论两种支撑管术中的效果均良好，是泪道理想支撑材料；但在手术时间和患者舒适度方面比较，RS型泪道再通管优于泪道引流管。Objective To compare the application efficacy between RS-type lacrimal intubation and tear duct drainage stent in the anastomosis of lacerated lacrimal eanaliculus. Methods The data of 78 eyes of 78 cases with lacrimal eanalicular laceration were collected. All cases were randomly divided into 2 groups. Group A,38 eases were treated with RS-type lacrimal stent and 40 cases were treated with drainage stent in group B. All patients were removed the stent 3 months after surgery. The differences of operative time, intraoperative comfort, efficacy and the rate of complications between the two groups were compared. Results The operation time of group A was （61.71 _＋ 8.79） rain, and group B was （68.25 ± 7.56） min. The difference was statistically significant （ t = 3. 527 ,P = 0.001 ）. The intraoperative pain score of group A was 3.58 ± 0.83 and 4.20 ＋ 0.97 of group B （ t = 3. 044, P = 0. 003 ）. The total rate of complications was 20.0% in group B and 15.8% in group A （χ^2 =0. 235 ,P =0. 656）. The postoperative evaluation was made when removed the stent 3 months after operation. The total effective rate of group A was 92.1% , while that of group B was 90.0% （χ^2 = 0. 106, P = 0. 752）. Conclusion Applications of two stent in surgery are effective, which are ideal supporting materials. However, RS-type lacrimal stent is superior to lacrimal duct drainage stent with short operation time and good intraoperative comfort.
