小梁切除术巩膜瓣两种缝合法的效果比较

摘　　要：目的比较小梁切除术三角形巩膜瓣两种缝合法的效果。方法急性闭角型青光眼小梁切除术67例（82眼），随机分为两组：A组，33例（41眼）；B组，34例（41眼）。手术中A组巩膜瓣缝合3针；B组巩膜瓣缝合2针。术后随访12个月，对前房及眼压恢复情况进行比较。结果术后l周，A组浅前房1眼，B组的10眼，差异有统计学意义（P=0．002）。术后3周A组眼压中位数为12．53mmHg高于B组的10．41mmHg（1mmHg=0．133kPa），差异有统计学意义（P=0．000）。结论小梁切除术三角形巩膜瓣3针缝合法较2针缝合法前房易形成，眼压稳定性好。Objective To compare the efficacy of two kinds of suturing of triangular scleral flap in trabeculectomy. Methods The data of 82 eyes of 67 cases with acute angle closure glaucoma were collected and all cases were divided into 2 groups. Group A, 41 eyes of 33 cases received 3 stitches of scleral flap and group B, 41 eyes of 34 cases received 2 stitches of scleral flap. The follow-up period was 12 months, and the deep of anterior chamber and intraocular pressure were compared between the two groups. Results At 1 week after operation, the shallow anterior chamber occurred in 1 eye in group A and 10 eyes in group B, and the difference was statistically significant （ P = 0. 002 ）. The mean intraocular pressure in group A was （12.53 ＋0.50） mmHg and （10.41 ＋ 1.08） mmHg in group B （1 mmHg =0. 133 kPa） at 3 weeks after operation. The difference was statistically significant （ P = 0. 000 ）. Conclusion Trabeculectomy with 3 stitches of triangular scleral flap is easy to form anterior chamber and stabilize intraocular pressure.
