摘　　要：医疗资源分配不均的问题日益突出,推进医疗改革、助力医联体建设迫在眉睫,为使我国基层护理人员能力与"医院-社区-家庭"联动的延续护理模式相匹配,本文介绍导师制模式下社区糖尿病管理护士培养的实践,旨在为提升社区护士慢性病管理能力提供一定参考依据。The problem of uneven distribution of medical resources is becoming more and more serious. It is urgent to push forward the reform of medical treatment and the construction of medical partnerships. In order to match the ability of primary nursing staff in our country with the transitional care mode of ＂hospital-community-family＂ linkage, the purpose of this article is to introduce the practice of community diabetes management nurses＇ training based on the mode of tutorial system, and to provide some reference for improving the management ability of chronic diseases of community nurses.
