单层连续内翻缝合法在腹腔镜下子宫肌壁间肌瘤剔除术中的应用

摘　　要：目的探讨单层连续内翻(棒球式)缝合法在腹腔镜下子宫肌壁间肌瘤剔除术中的应用,为大子宫肌瘤挖除术后创面缝合方法的选择提供参考。方法回顾性分析2016年1月~2017年12月在我院接受腹腔镜下子宫肌壁间肌瘤剔除术的40例患者,根据缝合方法的不同分为观察组和对照组,每组各20例。观察组采用单层连续内翻缝合止血法缝合子宫创面,对照组采用分层缝合法缝合子宫创面。比较两组的手术时间、术中出血量、手术前后血红蛋白差值。结果观察组的手术时间短于对照组,差异有统计学意义(P<0.05);两组的出血量比较,差异无统计学意义(P>0.05)。观察组手术后血红蛋白差值少于对照组,差异有统计学意义(P<0.05);观察组术后肛门排气时间短于对照组,差异有统计学意义(P<0.05)。结论腹腔镜子宫肌壁间肌瘤剔除术患者,应用单层连续内翻缝合法较分层缝合法更有优势及实用性。Objective To investigate the application of single-layer continuous inversion（baseball） suture in laparoscopic intramural myomectomy, so as to provide reference for the selection of wound suture method after excision of large uterine fibroids. Methods 40 patients who were given laparoscopic intramural myomectomy in our hospital from January 2016 to December 2017 were retrospectively analyzed. According to the different suture methods, they were divided into the observation group and the control group, 20 cases in each group. Patients in the observation group were given single-layer continuous inversion suture and hemostatic method to suture the uterine wound. In the control group,the uterine wound surface was sutured by layered suture. The operation time, intraoperative blood loss, and hemoglobin difference before and after surgery were compared between the two groups. Results The operation time in the observation group was shorter than that in the control group, the difference was statistically significant（P〈0.05）; there was no statistically significant difference in the amount of bleeding between the two groups（P〉0.05）. The difference in hemoglobin after surgery in the observation group was lower than that in the control group, the difference was statistically significant（P〈0.05）. The postoperative anal exhaust time in the observation group was shorter than that in the control group, the difference was statistically significant（P〈0.05）. Conclusion In the patients with laparoscopic intramural myomectomy, the application of a single-layer continuous inversion suture method is more advantageous and practical than the layered suture method.
